Dramatic changes in healthcare services have intensified practitioner's efforts to access and use healthcare information to determine more efficacious approaches to patient problems. HL7 (Health Level Seven) organization has been developing healthcare messaging standards for the representation of clinical documents. HL7 SIG activities include developing SML/DTD and XSL.
Despite a great deal of effort paid in digital on-line healthcare services, little development has been reported in nursing area. International Council of Nurses(ICN) in an attempt to bring a uniform nursing vocabulary into a standardized Unified Nursing Language System(UNLS).
The patient care activities are recorded in various forms of nursing documents. Among them , the nursing process document contains the core nursing process describing nurses' knowledge about patient care more than any other documents. Motivated by that, we focus on developing a digital on-line nursing process document as the goal of this research. The data elements of Nursing Process document include general patient data, nurse data, nursing process knowledge during patient care service, etc.
In this research, we develop web-based digital documents for nursing process by designing an XML/DTD and style sheets. Applying XML technology the digital representation and management for nursing process according to the recent nursing vocabulary standards specified by ICNP, XML documents create a set of tags of nursing knowledge. XML/DTD for nursing process document allows them to codify the vocabulary and syntax that users(i.e. nursing professionals) wish to use. By doing so, it is possible to search nursing vocabularies from database and display them in a form meaningful to nursing professionals. It also enables electronic exchange of information among different healthcare systems. We store DTD elements into a database and represent them tin XML document form through database queries. Databases tailored to manage XML data elements allow new opportunities for storage improvement and the manipulation of data. For a large application with a large amount of nursing data, this database approach would be more efficient and less expensive. Once a DTD of nursing process document is defined, we can organize nursing knowledge into style sheets to easily reformat the document automatically on various devices. In other words, we can use XML style sheets for data exchange and easy document form generation regardless of document structures that are different from system to system.
The successful design and implementation of XML document representation of nursing process will add strength to the Internet-based healthcare services. Also, it will stimulate the researches and developments in areas of digital nursing information systems and services.
의료환경에 있어서의 급격한 변화는 실무자로 하여금 환자문제에 대해 좀더 효율적으로 접근하고, 의료정보를 이용하기 위한 노력을 강화시키고 있다. HL7 (Health Level Seven)은 의료문서의 표현을 위한 보건정보전달의 표준을 개발하고 있다.
디지털 온라인 보건서비스에 대한 이러한 많은 노력에도 불구하고 간호 분야에서는 개발에 대해 보고된 바가 적다. 최근에 International Council of Nurses(ICN)은 간호어휘를 단일화된 간호언어체제로 만들기 위한 노력으로 International Classification of Nursing Practice(ICNP)를 개발하고 있다.
간호 활동은 다양한 간호문서의 형태로 기록되고 , 그 중 간호과정기록지는 다른 문서에 비해 환자 간호와 관련된 간호사의 지식을 표현할 수 있는 간호과정을 포함하고 있다. 본 논문에서는 이러한 이유로 인해 XML/DTD와 Style Sheet를 이용하여 간호과정을 위한 웹 기반의 디지털 문서를 개발하였다. ICNP로 정의된 간호어휘표준을 이용하여 간호과정을 위한 디지털 문서에 XML 기술을 적용하는 것은 실무자가 사용할 수 있는 어휘와 의미를 코드화 할 수 있도록 해준다. 또한 다른 병원체제간의 전자정보교환을 가능하게 해준다. 본 연구에서는 XML 데이터베이스에 DTD 요소들을 저장하고, 데이터베이스 질의를 통하여 XML 문서로 표현하였다. XML 데이터에 적합한 데이터베이스는 데이터의 조작을 개선할 새로운 기회를 제공한다. 다양한 응용에 있어서 데이터베이스 방식의 접근은 효율적이고 비용 절감의 효과가 있다. 간호 문서의 DTD가 정의되면, 다양한 장치를 통해 문서를 자동적으로 다시 만들 수 있는 Style Sheet으로 간호지식을 다룰 수 있다. 다시 말하며 XML Stylesheets를 이용하여 서로 다른 시스템에서의 문서형태와 무관하게 데이터교환과 문서를 생성할 수 있다.
간호과정을 XML문서로 표현하는데 있어서의 성공적인 설계와 개발은 인터넷 기반의 의료 서비스를 증진시킬 수 있다. 또한 디지털 간호정보시스템과 서비스 영역에 있어서의 연구와 개발을 촉진할 수 있다.
