As medical imaging and computer technologies advances rapidly, computerized analysis of medical images is becoming increasingly important. In particular, analysis of tubular objects in a human body is of concern to various medical applications. In many algorithms for dealing with tubular objects, a centerline has been widely engaged as an important tool due to its efficient expressivity of tubular structures in qualitative and quantitative analyses. Inappropriate use of a centerline, however, often leads to performance degradation. This thesis presents two novel centerline-based approaches aiming at two medical applications: Virtual colonoscopy and vascular abnormality quantification. First, in virtual colonoscopy, or a computer-aided screening modality for early detection of colonic polyps, the minimization of blind areas is a critical issue for accurate and effective diagnosis. Although the centerline is useful for describing the shape of an object, it does not always provide the optimal fly-through path in terms of minimum blind area. In practice, considerable blind areas can be produced by adopting a centerline as a path, especially in areas of high curvature. This thesis presents a novel centerline-based approach to the minimization of blind areas by incorporating surface information into a path-planning process. Contrary to conventional schemes, view positions and their corresponding view directions are jointly determined to increase the visibility of surface area. In addition, a new iterative pathsmoothening scheme is proposed to provide comfortable views without degradation of visibility performance. For the evaluation of the proposed algorithm, we first attempt to quantify the overall observable area on the basis of the temporal visibility measure that quantifies the minimum interpretation time of a human observer. The experimental results show that proposed algorithm improves visibility coverage and also significantly reduces the number of blind areas of a clinically meaningful size. Virtually-generated endoscopic videos show that the proposed algorithm can provide centered and comfortable views of colonography.
This thesis also deals with quantification of blood vessel abnormality. Given a segmented vessel boundary, quantitative analysis of abnormal structures can aid radiologists in choosing appropriate treatment and/or apparatuses. A centerline has been successfully applied to generate cross-sections of vessel lumen, quantify their diameters, and the length of an abnormal region. Thereby, segmentation and centerline extraction have been of major concerns for more accurate quantification. It has been reported, however, that the centerline of a vessel boundary sometimes have a bad influence upon the measurement of length and diameter in a clinical sense, and this is mainly due to the existence of locally high curvature of the centerline at an abnormal region. To eliminate the unwanted local curvature from the centerline, an active-tube-model is adopted to appropriately deform the centerline, by registering a deformable tube model onto the abnormal region of vessel lumen. For accurate registration, a region-based approach is introduced to the active tube model, in which energy functional is redefined adaptively according to region types; normal, stenotic, and aneurysmal regions. For reliable region classification, a complementary geodesic distance field is newly devised to generate cross-sections that are non-intersecting and robust to local geometry of vessels. The proposed algorithms are validated on a number of 3-D computer-generated phantoms and clinical datasets acquired from eight patients and are expected to provide exact volume measures of aneurysmal or stenotic regions for endovascular surgery planning.
최근 CT, MR 등 의료 영상 장치와 컴퓨터 처리 능력의 급속한 발달로 인해 고해상도의 삼차원 영상 처리 및 분석 기법 등이 중요해지고 있다. 특히, 인체를 이루는 장기 중 대장, 소장, 각종 혈관 등 다양한 종류의 관 모양의 조직 및 장기의 삼차원 영상은 그 임상적 활용가치가 높아 이들 영상의 처리 기법에 관한 연구가 활발하다. 복잡한 관모양 구조를 효율적으로 표현하는 중심선(centerline) 기반의 표현 기법은 그 단순성 때문에 영상의 정성적, 정량적 분석을 위한 도구로서 널리 사용되어 왔다. 그럼에도 불구하고 임상적인 응용에 따라서는 이를 단순 적용할 경우 그 효율성과 정확성이 크게 저하될 수 있음이 지적되어 왔다. 이 학위논문에서는 대표적 정성적 분석 방법으로 알려진 가상 대장내시경(virtual colonoscopy) 기법과 혈관조영영상(angiogram)을 기반으로 한 비정상 혈관의 정량화 기법 등에 적용 가능한 새로운 중심선 기반의 접근법을 소개한다.
먼저, 가상 대장내시경은 대장 용종(polyps)의 조기 발견을 위한 기법으로서 실제 내시경을 수행하지 않고도 고해상도의 3-D CT 영상을 기반으로 이를 가상 카메라가 일정한 경로를 따라 대장 내부를 비행하면서 실제와 유사한 내시경 영상을 합성하여 진단하는 방법을 말한다. 이를 위해 대장의 중심선을 자동 추출하여 카메라의 경로로 이용할 수 있다. 대장 내부 중심선은 표면으로부터 멀리 떨어져 있어 관찰이 용이하고 카메라와 대장의 외벽간의 충돌을 방지할 수 있다는 장점 때문에, 최근 수 년간 중심선을 정확하고 빠르게 구현하는 다양한 알고리즘에 대한 연구가 활발하였다. 그러나 가상 대장내시경의 정확성과 효율성을 극대화하기 위해서는 가상 카메라 경로의 비가시 영역을 최소화 하는 것이 중요한데 대장의 중심선은 이러한 관점에서 항상 최적의 경로로 볼 수 없다. 실제로 굴곡이 심한 부위에서 중심선을 경로로 한 경우 상당한 비가시 영역이 발생됨을 발견하였다. 이 학위논문에서는 중심선 정보뿐만 아니라 대장 표면의 정보도 함께 고려하여 얻는 경로를 이용함으로써 비가시 영역을 최소화 하는 새로운 기법을 제안한다. 이를 위해 기존 기법과 달리 관찰 위치 및 방향 벡터를 동시에 최적화 하였으며, 가시성능의 저하없이 안정적인 영상을 생성하기 위해 반복적 경로평활화 기법을 제안하고 이를 적용하였다. 제안된 알고리즘의 성능평가를 위해 관찰자의 최소 인지 시간을 반영한 시간적 가시성(temporal visibility)을 정의하고 이를 기준으로 비가시 영역을 측정하였다. 실험결과를 통해 제안된 경로 생성 기법이 가시면적을 향상시킴으로써 임상적으로 의미있는 크기의 비가시 영역이 수를 크게 줄일 수 있음을 확인하였다. 또한 경로를 따라 렌더링을 수행하여 가상내시경 비디오를 생성한 결과 제안된 기법을 적용한 경우 기존보다 매우 안정적인 영상을 얻을 수 있었다.
이 논문은 또한 3-D 혈관강조영상에서 비정상 혈관 영역의 정량화 기법을 다룬다. 분할된 이진 혈관 영상을 기반으로 비정상 혈관모양의 정량적 해석을 통해 임상의로 하여금 보다 적합한 치료법과 수술 도구를 선택하는데 도움을 줄 수 있다. 특히 중심선 기법은 혈관 단면의 지름 및 혈관의 길이 등의 정량적 측정하여 혈관 내 치료법(endovascular treatment) 등에 적용 가능하다. 이와 관련하여 혈관의 분리 기법(segmentation)과 그 중심선을 추출하는 연구가 주류를 이루어 왔다. 하지만 최근의 연구에서 이미 지적 되었듯이 비정상 혈관 영역의 형태학적 특징에 의해 혈관의 중심선은 국부적인 굴곡 현상을 동반하며 이 때문에 직접 정량 분석에 이용하는 경우 부적절한 결과를 얻을 수 있다. 따라서 이 학위논문에서는 중심선에 존재하는 원치 않는 국부적 굴곡의 제거를 위해 능동 관 모델(active tube model)에 기반한 중심선 변형 기법을 제안한다. 이는 변형 가능한 관 모델을 혈관 영상에 정합함으로써 중심선을 평활화 할 수 있다는 기본 개념에 근거한다. 정확한 정합을 위하여 영역정보에 기반한 능동 관 모델을 제안하였으며, 혈관 영역을 모양에 따라 정상, 협착(stenotic), 동맥류(aneurysmal) 영역으로 분리하고 영역에 따라 적응적으로 에너지 함수를 정의하였다. 효과적인 영역의 분리를 위해 단면 기반의 접근법을 채택하였으며 혈관의 국부적인 모양 변화에 민감하지 않고 상호 교차하지 않는 단면의 생성을 위해 상보적 최단 거리필드(complementary geodesic distance field) 개념을 제안하였다. 마지막으로 정합된 관 모델과 혈관 영상의 차영상을 이용하여 동맥류 영역을 분리하고 해당 부피 정보를 추출하여 혈관 내 치료법에 적용할 수 있는 가능성을 보였다. 제안된 기법의 검증을 위해 3-D 팬텀 모델과 실제 환자로부터 얻은 3-D 회전 디지털 감산 조영 영상(rotational digital subtraction angiography)에 적용하여 결과를 확인하였다.
