This paper deals with the qualitative human error analysis scheme for the analysis of human errors in nuclear power plant. The proposed scheme is based on Rasmussen's seuential error model and Hollnagel's classification scheme. For the structural analysis, this scheme is developed on the basis of cognitive model, which contains 4 stages of observation, interpretation, planning, and execution.
Because sequential error model lacks a clear distinction between manifestations and causes, the cogintive model is divided into two parts of external error mode(manifestation) and internal error mode(causes). The former is composed of execution, and the latter is composed of other stages. This distinction is proposed by Hollnagel. Error causes are divided into two parts of cognitive error causes and physical error causes for the exact analysis of error causes.
This scheme is composed of 4 dimensions : external error mode, internal error mode, cognitive error causes, physical error causes. This scheme provides hierarchical structure by linking related error modes with error causes.
This paper concludes by providing the case study of a accident in nuclear power plant in 1984.
