The current medical robot systems are too expensive, occupies a large space, and requires a separate operating room, which is difficult to supply to small and medium hospitals. Also, large hospitals can only have a limited number of robot systems. The bed-mounted laparoscopic surgical robot system minimizes the occupied volume by placing the base frame under the surgical bed. The bed mounted robot system developed by the KAIST research team which name is ‘APOLLON system’ aims to be distributed to small and medium-sized hospitals through miniaturization and modularization.
This paper aims to increase the miniaturization benefits of bed-mounted surgical robotic systems and their clinical applicability. The bed-mounted system approaches the patient from the side, so that the width of the robot becomes a constraint on placing the trocar port. To solve this problem, the gravity compensation mechanism that can reduce the width and size of the robot, and optimization algorithm that finding optimal port and pose of robot system was researched.
현재의 복강경 로봇 시스템은 가격이 지나치게 높고 점유공간이 커 별도의 수술 방을 필요로 하여 중∙소형 병원에 공급되기 힘들고, 대형 병원에서도 제한된 숫자의 로봇 시스템만을 보유할 수 있다. 침대 장착형 복강경 수술 로봇 시스템은 로봇의 베이스 프레임을 수술 침대 아래에 두고, 로봇을 환자 바로 옆에 두어 위치 결정에 요구되는 동작 영역을 줄여 로봇이 점유하는 부피를 최소화한다. 카이스트 연구팀이 개발한 아폴론 시스템은 침대 장착형으로, 궁극적으로 소형화 및 모듈화를 통해 중∙소형 병원에 보급되는 것을 목표로 한다.
본 논문은 침대 장착형 복강경 수술 로봇 시스템에서 소형화의 장점을 높임과 동시에 실제 임상에 적용을 위한 연구를 포함한다. 침대 장착형 시스템은 환자에 측면으로 접근하며, 로봇의 폭이 트로카 포트를 배치하는데 큰 제약이 된다. 이를 해결하기 위해 로봇의 폭과 크기를 줄일 수 있는 중력 보상 메커니즘 연구와 로봇의 폭을 제한 조건으로 갖는 트로카 포트 및 로봇 암 배치 최적화 연구를 수행했다.
