As a solution for immediately suppressing involuntary motor response such as spasticity in stroke patients, electrical nerve blocking technique with mid-frequency alternating current has been studied. However, in the case of the transcutaneous electrical stimulation rather than with the implantable electrode, there are difficulties in clinical use such as unintended muscle contraction and pain. Former studies have figured out the mechanism of electrical nerve block by computational modeling, but do not provide solutions to clinical problems. In this study, we developed a simulation model of transcutaneous electrical stimulation that combines electric field computation and stimulation model of neuromuscular system. We can conduct the parametric study with stimulation waveform, location of electrodes and body electrical characteristics to optimize electrical nerve block in clinical usage. As a result through developed model, it was confirmed that transcutaneous mid-frequency alternating current blocked the action potential of neuron but generated offset on membrane potential which results in incomplete block of muscle contraction.
뇌졸중 환자에게서 나타나는 경직과 같은 불수의적인 운동 반응을 즉각적으로 억제하기 위한 방법으로 중주파 교류 전류를 통한 전기적 신경 차단 기법이 연구되고 있다. 그러나 삽입형 전극이 아닌 피부 부착형 전기 자극의 경우 의도치 않은 수축과 통증이 발생하는 등 임상적 사용에 어려움이 있다. 기존 연구에서는 신경의 모델링을 통해 전기적 신경 차단술의 메커니즘을 밝혔으나, 경피 전기 자극에서 발생하는 문제에 대한 해결책을 제시하지 못하고 있다. 본 연구에서는 유한요소법을 통한 체내 전기장 계산과 근신경계 자극 모델을 결합한 경피 전기 자극 시뮬레이션 모델을 개발하였다. 개발된 모델을 통해 전기 자극의 파형과 전극 위치, 체내 전기적 특성 등의 매개변수 연구를 통해 임상적 환경에서 전기적 신경 차단술의 최적화를 수행할 수 있다. 모델을 통한 시뮬레이션 결과, 중주파 경피 자극으로 인해 뉴런의 활성 전위는 차단되지만 오프셋이 발생하여 불완전한 수축 억제가 발생함을 확인하였다.
