Stroke is a neurological symptom caused by explosion or block of blood vessels, which 9% of post-stroke patients completely recover, but 73% of patients experience sequela like paralysis, spasticity, dementia and aphasia. Most of the post-stroke experience clenched hand deformity due to hypertonic flexor muscles caused by spasticity and weakness of extensor muscles. Since recovery of the motor function through rehabilitation has a limit, more than half of patients in the chronic stage still could not use their affected hand in activities of daily living(ADL), requiring assistance. Many orthoses have developed for assistance. Passive devices mainly focus on passively assisting hand opening by placing springs along dorsal side which are connected to fingertips and does not assist grasping. Active devices assist flexion and extension by using cable actuated by motors, but they tend to have large actuating module limiting the portability. In this study, we developed a 3D printed soft hand orthosis based on finite element method that passively assist opening by placing structure on the palmer aspect and actively assist closing by applying twisted string actuation that allow enlarged force with compact design. A thickness profile of the orthosis was optimized based on the patient’s metacarpophalangeal and proximal interphalangeal joint stiffness to open the hand and minimize the actuation force for conducting pinching. Total six patients were recruited who were in the chronic stage. The affected hand was opened as desired and they could pinch ordinary objects that could be seen in ADL. The proposed orthosis is expected to help the patients to conduct bimanual activities in daily living.
뇌졸중은 혈관이 막혀서 발생되는 신경학적 증상으로써 발생 시 9%는 완치되지만 73%는 마비, 경직, 치매 그리고 언어장애를 겪게 된다. 그 중 경직은 손에서도 나타나는데 손의 굴근이 과도하게 긴장되어 손이 펴지지 않아 손을 쥐고 있는 전형적인 형태를 보인다. 재활을 하더라도 반 이상의 뇌졸중 환자가 일상생활에서 경직된 손을 사용하지 못하고 따라서 보조가 필요하다. 지금까지 크게 두 종류의 보조기가 개발되었다. 손을 수동적으로 펴주는 보조기는 용수철과 같은 탄성체를 이용해 손을 펼 수는 있지만 그랩이나 핀칭과 같은 동작을 보조하지는 못한다. 모터를 통해 굽힘과 폄을 보조하는 보조기의 경우, 환자의 경직을 극복하기 위해서는 큰 구동부를 필요로 하기 때문에 휴대성이 떨어지는 문제가 존재한다. 본 연구에서는 탄성재(Thermoplastic Polyurethane(TPU))의 3D 프린팅과 모터를 통한 줄 꼬임 메커니즘을 사용하여 환자 손의 폄과 굽힘 동작을 모두 보조하는 보조기를 개발하고자 한다. 환자 손의 엄지, 검지, 중지의 손바닥 부분에 유한요소기법을 통해 설계된 탄성 재질의 구조물을 고정함으로써 손을 수동적으로 펴고 줄을 구조물에 라우팅 시켜 당김으로써 탄성재를 굽혀 손의 굽힘 동작을 구현한다. 환자마다 손의 경직도와 형태가 다르기에, 3D 스캔 기반, 각 관절 경직도에 따른 최적의 보조기 두께 프로파일을 제시하는 알고리즘을 개발함으로써 핀칭을 위한 모터가 필요한 힘을 최소화 하면서 착용 시 수동적으로 환자의 손을 펴고자 한다. 총 6명의 만성 단계의 뇌졸중 환자를 모집하였고 맞춤형 보조기 생성 및 착용 시킨 결과 환측의 손이 원하는 만큼 펴졌고 일상생활 속 다양한 물건 또한 잡을 수 있었다. 개발한 보조기가 환자의 일상생활 속 양손을 사용한 동작을 보조할 수 있을 것이라고 기대된다.
