Pericyte dropout during breakdown of the blood?retinal barrier (BRB) is a major hallmark ofdiabetic retinopathy (DR), but it is unclear how pericyte dropout contributes to the progressionof DR. In this study, I show that PDGF-B/PDGFR$\beta$ signaling, which is essential for BRBbuildup in growing retinal vessels, is dispensable for maintaining BRB integrity in adult stableretinal vessels. Moreover, selective pericyte depletion in adult stable retinal vessels surprisinglydid not show phenotypes of BRB disintegration. However, the vessels became susceptible tobe leaky by external VEGF-A stimulus, which could be mediated by up-regulating vasculardestabilizing factors including Angiopoietin-2 and VEGFR2 through transcriptional activationof FOXO1 in unstable endothelial cells. Accordingly, either blocking Angiopoietin-2 oractivating Tie2 greatly ameliorated progression of BRB breakdown. Together, althoughpericyte coverage in quiescent endothelial cells is expendable, its impairment leaves themicroenvironment vulnerable to external stimuli that aggravate DR progression.
망막 혈관으로부터 혈관주위세포가 소실되는 것은 혈액-망막 장벽이 파괴되며진행하는 당뇨망막병증의 초기 변화로 알려져 있다. 하지만, 혈액-망막 장벽에대한 혈관주위세포의 역할에 대해서는 아직 잘 밝혀져 있지 않다. 본 연구를 통해PDGF-B/PDGFR$\beta$ 신호전달체계가 생쥐의 생후 자라나는 망막 혈관의 혈액-망막 장벽 기능에 중요하지만, 안정화된 성체 생쥐의 망막 혈관에서는 중요하지않음을 확인하였다. 성체 생쥐의 안정화된 망막 혈관에서 혈관주위세포가선택적으로 소멸될 경우 그 자체로는 혈액-망막 장벽이 파괴되지 않았지만, 외부VEGF-A의 주입에 의한 혈액 누출과 염증 반응이 의미 있게 증가하여 VEGFA에대한 혈관의 반응성 조절에 혈관주위세포가 중요함을 확인하였다. 이는혈관내피세포에서의 FOXO1의 활성 증가와 이로 인한 Angiopoietin-2 나VEGFR2 와 같은 혈관내피세포의 불안정성과 관련된 물질들의 생성 증가로 인한것이었다. 나아가, Angiopoietin-2 의 활성을 억제하거나 Tie2 수용체의 활성을증가시킴으로써 혈관주위세포의 탈락으로 인한 혈액-망막 장벽의 파괴를 막을 수있었다. 즉, 안정 상태의 망막 혈관에서 혈관주위세포의 탈락 자체는 혈액-망막장벽의 안정성에 영향을 주지 않지만, VEGF-A 와 같은 외부자극에 더욱민감하게 반응하게 함으로서 혈액-망막 장벽의 파괴를 가속화시키고, 이러한과정은 당뇨망막병증의 진행에도 중요하게 적용될 것으로 생각된다.
