Kidney transplantation (KT) is successfully carried out with immunosuppressive treatment of recipients. However, recipients receiving immunosuppressants are at great risk of viral infections or cancer due to life-long duration of the treatment. Therefore, studies have been performed for alternative protocols in attempt to overcome such complications, including infusion of regulatory T cells (Tregs) and administration of blocking antibodies against co-stimulatory molecules. Among those is to perform bone marrow transplantation (BMT) simultaneously with KT (BMTKT) to induce mixed chimerism in the recipient and establish immune tolerance to the grafted organ.
In the current research, we conducted BMTKT by recruiting six patients who are living donor KT recipients and each have a one-haplotype match donor. In these patients, we analyzed the recipients’ peripheral blood mononuclear cells collected at various time-points throughout the transplantation period to investigate the dynamics and phenotypes of various immune cell populations including memory CD4^{+} and CD8^{+} T cells, and Tregs and subsets within the population.
As of result, we found the ratio of CD45RA^{-}Foxp3^{bright} Tregs (effector Tregs with suppressive function) to CD45RA^{-}Foxp3^{dim} Tregs (non-suppressive, pro-inflammatory cytokine producing cells) as a unique biomarker of immune tolerance. In addition, there was a difference in the frequency of Tregs in mixed lymphocyte reaction (MLR) assay between patients experienced rejection episodes and those who have not. Collectively, our study reports that Tregs play a role in inducing donor-related immune tolerance in BMTKT patients.
신장이식은 만성 신장 질환의 환자들에게 새로운 삶을 제공하는 가장 중요한 치료 방법으로 자리 잡고 있다. 그러나 신장이식 전이나 이식된 장기가 기능을 하는 동안에는 면역억제제를 지속적으로 복용해야 하는 어려움이 있다. 면역억제제의 개발로 인하여 이식 거부 반응을 억제하여 신장 이식의 성적이 크게 향상됨에도 불구하고 장기간 복용으로 인한 부작용으로 신장 기능의 상실, 약화된 면역 시스템으로 인한 기회 감염 노출의 확률이 높아지고 암의 발생율도 높아질 수 있다. 현재 이러한 부작용을 줄이고자 이식 후 면역억제제를 중단할 수 있는 방법으로 조절 T 세포와 면역 세포 자극의 차단 등을 이용한 다양한 면역관용 유도를 향한 임상 실험이 이루어지고 있다.
본 임상 실험은 6명의 생체 신장이식 환자에서 2개이상의 조직적합성이 일치한 환자에서 시행 되었고 기증자의 골수 이식과 신장이식을 동시에 진행하여 수혜자 안에서 기증자의 골수 이식으로 유도된 일시적인 donor chimera를 이용하여 면역관용을 도모하고자 하였다. 6명중 3명의 환자는 성공적으로 면역관용이 유도 되었고 3명은 거부반응을 보였다. 이처럼 골수 및 신장 이식을 시행한 환자들은 계속적으로 시간대 별로 환자들의 말초혈액 림프구를 냉동 보존하였다. 본 실험에서는 이식거부 반응에 조절 T 세포가 관여를 한다는 것을 수혜자와 공여자의 혼합 림프구 반응 (Mixed lymphocytic reaction) 실험을 통해서 조절 T 세포의 중요성을 확인 하였고 냉동 보존된 림프구에서 다시금 면역관용 유도에 있어 조절 T 세포의 역할을 확인하였다. 면역관용이 유도된 환자의 경우 거부 반응을 보인 환자와는 달리 혼합 림프구 반응 실험에서 기증자의 면역세포를 배양보조세포로 주었을 때 제 삼자의 면역세포를 배양 보조세포로 주었을 때 보다 조절 T 세포가 우세하게 관찰 되었다. 냉동 보존된 림프구에서는 면역 관용이 유도된 환자들에서는 면역 억제 기능이 활성화된 fraction-II 와 염증성 기능이 활성화된 fraction-III의 비율이 이식 후 지속적으로 높게 유지 됨을 관찰하였다. 더불어 조절 T 세포의 아형 중에 염증성 기능을 하는 fraction-III 조절 T 세포가 이식 거부 반응을 경험한 환자군에서 더 많음을 확인하였다.
이러한 결과들을 종합하여 볼 때, 면역억제기능을 보이는 조절 T 세포가 donor chimera 및 면역 관용을 유도하는데에 있어 중요한 역할을 한다는 것을 제시하고 있다.
