This thesis proposes a method to construct conceptual database schema from the analysis of business forms used in an enterprise. In order to describe the properties of forms that are useful in designing database, conventional form model is extended by database operations. From fields are selected by the operations defined between form fields. Based on the extended form model, a conceptual database design system is developed and implemented. The database design system provides graphical interface to designers in generating conceptual schema and generates database schema automatically more than conventional database design system.
