Thanks to the affluent medical resources and a well thought-out health insurance scheme, coupled with the development of high-level information technology and its corresponding national infrastructure, Korea meets the criteria to be a leading nation of telemedicine. However, ever since the demonstration of telemedicine in 1988, telemedicine has not been practiced successfully until the present-day and the debate over its nationwide implementation is yet to find resolution. The current situation, therefore, signifies the lack of acceptability of telemedicine from the medical professionals and the patients, who serve as purveyors and clients, respectively. This research understands that the main cause of this happening is due to highly possible discrepancy of the elements that make up the telemedicine system. Correspondingly, researches on the acceptability of telemedicine, including the correlation and understanding of the components of telemedicine, have failed to achieve completion. Current researches on the acceptability and the performance of telemedicine are mainly based on the external factors of the system, that is to say skills or the legal and political propulsion. This research carries out the three-dimensional methodology of foresight (time, space and field) with focus on the acceptance of telemedicine system in Korea by domestic medical professionals. Through this, the thesis points out the unique situations of the domestic telemedicine system with regards to factors such as problem definitions and stakeholder’s agreements by comparing Korea to the countries with developed telemedicine systems. Moreover, the medical professionals play an integral role in the value chain of the current telemedicine system; this research investigates how the professionals, who are particularly important in the introduction and the implementation of the telemedicine, respond negatively to the current telemedicine system imposed by the national healthcare policies. Thereafter, this research carries out forecasting of the short and medium term futures of domestic telemedicine through the three-dimensional foresight and concludes with scenario planning. By imposing profound solutions to solve the problems of the current telemedicine system, this thesis constructs a strategy for a sustainable telemedicine system in Korea.
한국은 잘 조성된 건강보험제도와 풍부한 의료자원 등 선진 의료시스템을 갖추고 있으며 높은 수준의 정보통신기술의 발달, 이와 관련된 기반 시설의 전국적인 구축으로 원격의료 선진국으로서의 요건을 갖추고 있다. 하지만, 1988년 원격의료의 시범사업 이후로, 현재까지 원격의료가 제도로써 시행되지 않고 여전히 시범사업과 정책 실행에 대한 논의만을 반복하고 있다. 이러한 상황은 국가 주도의 원격의료 서비스에 대한 의료공급자인 의료계 및 의료수혜자인 환자들의 수용성이 부족함을 의미할 수 있다. 그 핵심적인 원인은 원격의료 시스템을 이루는 구성요소들 간의 부조화가능성이 높으며, 그들 간의 상호 관계 및 이해를 바탕으로 한 수용성에 관한 연구는 충분히 이루어지지 못하고 있다. 현재의 원격의료의 수용 및 실행에 관한 연구는 시스템의 외적인 요인 즉, 기술 혹은 법적, 정책적인 추진을 위주로 연구하는 측면이 있다. 본 연구에서는 원격의료의 수용성을 중심으로 3차원적 미래예측 방법 즉, 시간, 공간, 분야에 따른 분석을 통해 원격의료 선진국들과 비교함으로써 그들과는 다른 문제정의, 이해관계자 간의 합의 등 한국 원격의료의 특수한 상황을 파악하였다. 또한, 현재 원격의료의 가치 사슬 중 도입과 시행의 중심에 있으나 수용에 부정적인 입장을 취하고 있는 의료계의 저항성을 분석하였다. 이후, 원격의료의 중·단기 미래를 예측하고 미래 설계 후 시나리오 기획을 시행하였다. 현재 한국 원격의료의 낮은 수용성에 대한 근원적인 해결방안을 제시하여 지속 가능한 원격의료를 위한 전략을 수립하였다.
