Optical coherence tomography (OCT) is a useful imaging method for in vivo tissue imaging with deep penetration and high spatial resolution. However, current application of OCT is limited to clinical imaging in ophthalmology and cardiology. Recently, a number of extensions of OCT have been developed for improving image contrast. Functional extensions of OCT will catalyze its usage in biomedical research for in vivo imaging of animal disease models and also in novel area of clinical diagnostics. Here, I sought to demonstrate the development of multi-functional OCT system (polarization sensitive OCT and quantitative angiographic OCT) and its application to cardiovascular or cerebrovascular disease models. First, a mouse model of myocardial infarction was evaluated by polarization-sensitive OCT (PS-OCT). Robust PS-OCT image processing, structural and functional imaging in vivo, and development of a novel method for myocardial fiber orientation was performed. Second, cerebral blood flow monitoring of a photothrombotic stroke model using photosensitizer (Rose Bengal) in a mouse cranial window will be performed with quantitative angiographic OCT (qAOCT). The preliminary results during the development of qAOCT, hemodynamic imaging of a cranial window model was introduced. Third, a swine model of coronary atherosclerosis and stent structure was assessed by high-speed intracoronary OCT. The high speed intracoronary OCT with prospective ECG triggering was developed. In vivo imaging in a beating swine heart presented 3D coronary microstructure in high quality without cardiac motion artifact. Taken together, functional extensions of OCT including PS-OCT, qAOCT and high speed intracoronary OCT and their application in animal disease models will widen our understanding of human cardiovascular and cerebrovascular diseases.
OCT는 깊은 투과력과 높은 공간해상도를 지니고 있어 생체내 조직 이미징에 매우 유용한 영상방법이다. 그러나 현재 OCT의 적용은 안과와 심혈관질환분야의 임상 이미징에 국한되어있다. 최근 수많은 OCT기능의 확장이 이루어졌으며, 이러한 기능적 확장은 OCT가 동물질환모델의 생체내 이미징을 통한 의생명연구, 임상진단분야의 새로운 영역에서의 활발하게 활용되는 것을 촉진시킬 것이다. 본 연구는 편광민감 (po-larization-sensitive) OCT, 정량적 혈관영상 OCT 등의 다기능 OCT 시스템의 개발 및 이를 이용한 심뇌혈관 질환 모델에서의 적용에 관한 연구 성과를 보여주고자 하였다. 첫째로, 편광민감 OCT를 통해서 마우스의 심근경색모델의 생체내 이미징 및 기능/구조의 평가에 관한 연구를 수행하였다. 정교한 편광민감 OCT 영상처리, 생체내 구조/기능적인 변화 평가, 심근섬유의 방향적 일관성을 평가하고자 하는 새로운 표현 방법의 개발 등이 이루어졌다. 둘째로, 혈류속도를 정량적으로 측정할 수 있는 정량적 혈관영상 OCT의 개발과 이를 뇌경색 모델에 적용한 연구를 수행하였다. 마우스의 두개골 윈도우 모델을 이용하고, 광민감제와 빛을 이용하여 국소적인 허혈성 뇌경색을 일으키고, 뇌혈류량 변화를 관찰하는 실험을 수행하였고, 본 연구에서 개발한 정량적 혈관영상 OCT가 넓은 범위의 뇌혈류량을 측정할 수 있고, 시간에 따라 변화하는 혈류량의 차이를 혈관단위로 영상화할 수 있음을 확인하였다. 뇌경색시 산소투여가 뇌혈류에 미치는 영향을 분석하였고, 과도한 산소투여가 근거가 있는 것인지에 대해 분석하였다. 셋째로, 초고속 관상동맥용 OCT를 개발하였고, 이를 통해 토끼대동맥 및 돼지 관상동맥에서 동맥경화 및 스텐트 미세구조의 3차원 영상화 결과를 나타내었다. 토끼 대동맥 영상을 통해 고해상도 3차원 혈관영상이 가능함을 구현하였고, 심장박동에 따라 움직이는 돼지 관상동맥에서는 심전도 신호에 따라 움직임이 적은 이완기에 빠르게 혈관영상을 획득하는 방법을 개발하였고, 이를 통해 심장 움직임에 의한 왜곡이 적은 3차원 혈관영상을 획득할 수 있었다. 종합적으로, 편광민감 OCT, 정량적 미세혈관영상 OCT, 초고속 관상동맥 OCT 등 OCT의 기능적 확장에 대한 연구를 수행하였고, 이를 동물질환모델에 적용함으로써 인간의 심혈관 및 뇌혈관 질환에 대한 이해를 넓혀나갈 수 있는 새로운 연구방법을 제시하였다.
