Digital Breast Tomosynthesis (DBT) is a new three-dimensional (3D) limited-angle tomography breast imaging technique that has the potential to significantly reduce camouflage effect of overlapping fibroglandular breast tissue that is a major inherent limitation for better lesion detection and identification in mammography. Since the radiologists need to interpret the large volume of DBT data, there may be chance of missing malignant lesion in breast cancer diagnosis. Thus, it is desirable to design computer-aided detection (CAD) system that aims at automatically detecting malignant lesions in DBT. Typically, for detecting clustered microcalcifications (MCs), there are two approaches. One approach uses reconstructed volume and the other one uses projection views. Each image has factors that make it difficult to detect the clustered MCs. In the reconstructed volume, MCs are distributed several slices and appear blurred. Thus, there is limitation to extract accurate features. In case of the projection views, signal to noise ratio (SNR) of MCs are low hence the sensitivity is generally lower than that of the reconstructed volume. In order to overcome aforementioned limitations, novel methods are proposed in this paper. Firstly, in order to resolve structural limitation and blur problem, maximum ray-tracing based feature extraction method is proposed that features are extracted from images that describe structural characteristics of the clustered MCs. Secondly, a novel preprocessing method in projection views is proposed. In the proposed method, MCs which are shown repeatedly in the projection views are emphasized, while random noise is suppressed. Lastly, a combined approach that fuse detection results from the reconstructed volume and projection views is devised. The combined approach is based on the fact that different approaches detect clustered MCs, while are likely to detect different false positives. Therefore, the proposed combined approach makes stronger and accurate detection results. Comparative and extensive experiments have been conducted on DBT data set provided by private hospital. Our results show that the proposed combined detection approach can improve the clustered MCs detection sensitivity with relatively small number of false positives, compared to the cases where detection approaches from reconstructed volume and projection views are separately used.
디지털 토모신세시스는 새로운 3차원 단층 유방 영상 기술이다. 디지털 토모신세시스는 기존에 2차원 유방조영술이 가지고 있는 제한 사항인 유방 조직의 중첩으로 인한 위장 효과를 개선할 수 있는 잠재력을 가지고 있다. 하지만 의료전문가가 진단을 위해 많은 양의 재구성 영상을 판독해야 하기 때문에, 유방암 진단에서 악성 병변을 놓칠 가능성이 있다. 따라서 디지털 토모신세시스 영상에서 자동으로 악성 병변을 검출하는 컴퓨터 지원 진단 시스템 개발의 필요성이 활발히 논의되고 있다. 일반적으로 유방암 병변인 군집 미세석회화를 검출하기 위해 두 가지 접근 방법이 있다. 한 가지 방법은 재구성 영상을 사용하며, 다른 하나는 투영 영상을 사용한다. 각 영상에는 군집된 미세석회화 검출을 어렵게 하는 요인이 존재한다. 재구성 영상에서는 미세석회화가 여러 슬라이스에 걸쳐서 나타나고, 미세석회화 주변에 블러(blur)가 나타나기 때문에 정확한 특징정보를 얻는데 한계가 있다. 투영 영상에서는 미세석회화의 신호 대 잡음 비 (SNR)가 낮기 때문에 일반적으로 재구성 영상보다 군집 미세석회화의 검출률이 낮다. 따라서 본 논문에서는 이러한 한계를 극복하기 위해 새로운 방법을 제안한다. 먼저, 재구성 영상에서 군집 미세석회화의 구조적 한계와 블러 문제를 해결하기 위해 3차원 관심영역 추출 후 최대값 투사 영상으로 변환하여 군집 미세석회화가 뚜렷이 나타나는 영상에서 특징정보를 추출하는 방법을 제안한다. 또한, 투영 영상에서 미세석회화의 낮은 신호 대 잡음 비 문제를 극복하기 위해 투영 영상간 반복적으로 나타나는 미세석회화는 강조하고, 랜덤한 패턴을 보이는 잡음은 제거하는 전처리 방법을 제안한다. 마지막으로 각 영상에서 군집 미세석회화 검출 결과를 결합하는 방법을 제안한다. 제안하는 방법은 서로 다른 군집 미세석회화 검출 알고리즘은 동일한 군집 미세석회화를 검출하지만 서로 다른 거짓 양성 (false positive)을 검출하는 것에 근거를 두어 서로 다른 알고리즘의 조합을 통해 더 강인하고 정교한 검출 결과를 얻는 것을 특징으로 한다. 제안하는 방법을 검증하기 위해 사설 병원에서 제공받은 임상 데이터를 사용해 비교실험을 수행하였다. 실험을 통해 제안하는 방법이 낮은 거짓 양성 수에서 높은 검출률을 얻음으로 재구성 영상과 투영 영상을 개별적으로 사용하는 기존 검출 방법보다 우수함을 보였다.
