In recent years, many approaches have been developed to detect biomolecules for diagnosis for various diseases. The success of these approaches largely relies on the integration of interdisciplinary knowledge of molecular biology, chemistry and physics. The detection system in molecular diagnostics demands high sensitivity and specificity by employing various materials and platforms such as microarray-based techniques and nanomaterials-based biosensor. Additionally, as all diagnostic devices and products are trending toward high throughput screening and miniaturization, electrical approaches as well as optic approaches come into the spotlight and its studies are vigorously progressed worldwide.
This thesis introduces the development of various diagnostic tools using molecular technology for clinical disease. The value and practicality of these diagnostic systems developed in this thesis is enhanced by applying the clinical samples.
First, DNA microarray for microbial pathogen detection was developed by considering high prevalence rate and/or difficulty of cultivation of pathogen. I directly determined the sequences of 23S ribosomal DNA (rDNA) and 16S-23S rDNA intergenic spacer region (ISR) for 19 bacteria with the limited or no availability of their sequences in public databases,facilitating the advance of various assays for pathogen detection. Employing bacteria-universal probes as well as species-specific probes in DNA microarray system enables to reduce false negative results by detecting all bacteria. As a result of the validation using 515 clinical isolates, the DNA microarray allowed efficient detection of bacterial pathogens with the specificities of 100% with sensitivities of 100% as well except for two pathogens. Furthermore, the detection of pathogen using DNA microarray was expanded up to 52 bloodborne pathogen, so far the largest number of pathogens. The microarrays which were applied to 112 blood culture positive samples, showed also successful identification of bloodborne pathogen. In addition, the good discrimination of the closely-related species using unique hybridization pattern showed the possibilities of novel pathogen detection.
Second, I have applied the strategy to the development of a type-specific diagnostic procedure for MRSA and VISA. The real time-PCR analysis reveals that methicillin and vancomycin resistance level affects the expression of 5 novel genes (fmtB, plc, MW2442, SAR2103 and SAV1976). The results presented here suggests novel target from comparative genomic analysis which might make a good biomarker for MRSA and VISA and could be used to develop a rapid molecular test for the detection of these strains.
Third, array-based S1 nuclease assay was devised by using the known ability of the S1 enzyme to act as a single-stranded DNA-specific endonuclease. I first determined the best condition for the assay by modulating the probe length and concentration, and the amount and treatment time of the enzyme. Under the optimal condition, it is observed that our system could detect DNAs regardless the distance from 5’florophore and probe. Also, this system was applied to pathogen detection, mismatched DNAs detection and genotyping, showing the possibility of practical usage in clinical diagnostics.
Fourth, I demonstrated the label-free DNA detection system using SWNT-based two terminal resistor. It is based on the conductance change of device combined with the recognition capacity of the DNA-DNA interaction. The use of SWNT without cumbersome separation process for semiconducting SWNTs enables to us ease the scale-up fabrication process. The DNA immobilization using amide reaction, covalent binding, allows us to strengthen the binding between DNAs and SWNTs as well as to preserve the pristine SWNT electrical property, which enhances the efficiency of the further hybridization. Furthermore, this biosensor has shown that it is useful as a label-free platform to identify pathogenic bacteria by applying the DNAs from reference strains, having the sensitivity of 10 cells per assay. Thus, these advantages of this system are believed to make SWNT-based electrical sensor a promising candidate for highly miniature chip and hand-held diagnostic electronics.
Molecular technologies have a significant role to play in clinical diagnosis although their adaptation will slowly replace conventional methodologies, which continue to be the cornerstone of modern methods. This thesis will facilitate the development and application of molecular technology in diagnostic field.
질병 진단은 치료에 있어 매우 중요하다. 특히, 인간의 생명과 직결되어 있는 질병의 경우 신속, 정확, 간편한 진단은 더욱 그 중요성을 더한다. 이러한 중요성 때문에 진단기술의 개발은 계속적으로 요구되어 왔으나, 아직까지도 감도의 향상이나, 신속성, 정확성, 신뢰성의 향상 등 해결해야 할 과제들이 많이 남아있다. 따라서, 질병진단에 있어서 보다 향상된 기술의 개발은 계속적으로 요구된다.
유전자 진단기술이란, 유전자 정보를 바탕으로 하여 질병 여부 등을 판정하기 위해 혈액, 척수액, 복막액 등 인체에서 유래하는 시료를 검체로 하여 특정 지표물질을 검출하거나 정량 분석하는 방법을 말한다. 1985년 Mullis에 의해 개발된 Polymerase Chain Reaction (PCR)과 사람을 비롯한 감염균 유전체지도의 완성이 유전자 진단 기술 발전의 계기를 제공한 이래, 다량의 유전정보를 고속으로 분석하는 것이 가능하게 됨으로써, 유전자 해독기술의 발전에 따라 이로부터 얻어진 유전자 정보들을 활용한 진단 기술의 개발도 가속화되고 있다. 유전자 진단 기술은 일회 검사로 다양한 병원균에 대한 감염 여부나 여러 개의 유전자를 한꺼번에 검사할 수 있기 때문에 진단의 정확성과 유용성을 향상시킬 수 있다. 이 기술은 현재 세계시장에서 약 20%의 연 평균 성장률을 보이며, 체외진단검사 시장 중 가장 빠른 속도로 성장하는 분야로 부각되고 있다. 올해는 30조 가량의 체외진단검사 시장 중 8%의 점유율을 보이고 있고, 2008년에는 13% 이상을 차지하며 매출규모는 5조 이상이 될 것으로 전망된다.
따라서, 이러한 유전자 진단기술을 이용하여 다양한 질병을 보다 빠르고 정확하고 간편하게 진단하기 위한 시스템을 개발하기 위해 다음과 같은 세부항목의 연구를 수행하였다.
첫째, DNA 칩 기술을 감염질환 진단 시스템 개발 및 임상에 적용함으로써, 실제 임상시스템에서 사용이 가능하도록 하였다. 본 연구에서는 임상적으로 가장 빈번하게 감염질환을 일으키는 52종의 감염질환 원인 균주를 선정하여 이의 목적유전자를 대상으로 현재 알려진 염기 서열을 확보하였고, 알려지지 않은 균주의 경우는 염기서열은 직접 분석함으로써 유전정보를 확보하였다. 또한, 균주를 동시에 증폭시킬 수 있는 공통 탐침과 균 특이 탐침을 디자인하여 진단용 DNA 칩을 제작하였다. 제작된 DNA 칩에 총 515개의 다양한 임상 검체로부터 추출한 임상 분리주와 112 개의 양성혈액배양액을 적용하였고, 그 결과로 약 92% 이상의 민감도와 100%의 정확도를 얻었다. 따라서, 본 연구에서 개발된 DNA 진단칩은 기존의 진단방법의 한계를 극복하여 보다 신속하고 정확하게 다양한 임상 검체를 쉽게 진단할 수 있어 병원균에 대한 감염여부를 한번의 검사로 알 수 있기에 정확한 조기 진단이 가능할 것이다.
둘째, 메치실린 및 반코마이신 항생제 내성 황색포도상구균 진단용 유전자 진단 기술을 개발하였다. 감염질환 원인 균의 부적절한 진단으로 의해 발생되는 항생제 오남용으로 항생제 내성 균주가 계속적으로 출현하고 있어 전세계적으로 큰 문제가 되고 있다. 본 연구에서는 항생제 내성 균주의 게놈 염기서열을 비교 유전체학 분석을 통해 새로운 목표 유전자들을 발굴하였고, 발굴된 유전자의 유용성을 확인하기 위해 표준 균주 및 임상 분리주를 적용하였다. 그 결과, 기존에 사용되어온 유전자에 비해 보다 정확한 균주 내성 유무를 확인할 수 있었다. 따라서, 본 연구에서 발굴한 새로운 유전자를 이용한 메치실린 및 반코마이신 항생제 내성 황색포도상구균 검출방법은 고감도로 황색 포도상구균의 내성여부를 검출할 수 있으며, 또한 기존에 사용되어온 mecA 유전자에 의존한 PCR 의 위양성의 오류를 최소화할 수 있을 것을 기대된다.
셋째, 효소 반응를 적용한 질병 진단기술을 개발하고자 한다. 유전자간의 특이결합과 더불어 단일가닥의 유전자만을 선택적으로 분해하는 S1 뉴클리아제효소를 도입하게 되면 형광물질과의 거리에 따른 형광세기의 감소영향을 전혀 받지 않으면서 보다 정확한 DNA의 결합여부를 확인할 수 있었다. 이 시스템의 유용성을 보다 증대시키기 위해 감염균 동정 및 SNPs 검출을 모델로 하여 높은 정확성을 보임을 증명하였다.
넷째, 나노 기술과 유전자 진단기술을 접목하여 보다 정확하고 민감하면서도 소형화된 질병 진단 시스템을 개발하고자 탄소나노튜브를 기반으로 한 바이오센서를 개발하였고, 전기적 신호를 통해 DNA 검출을 꾀하였다. 기판 위에 네트워크를 형성한 탄소나노튜브 소자위에 공유결합을 통해 단일가닥 DNA 를 고정화시키고, 이에 이중가닥을 형성하는 목표 DNA 또는 반응을 하지 않는 DNA 를 각각 적용한 후, 선택적으로 목표 DNA 를 정확하게 검출함을 컨덕턴스의 변화율을 통해 확인하였다. 또한, 개발한 나노 센서의 유용성을 확인하기 위해 감염균 동정용 DNA 를 적용한 결과, 정확하게 표적 균주를 동정함을 확인할 수 있었다.
본 연구들은 질병의 유무를 보다 값 싸고, 조기에 빠르고, 정확하며, 고속으로 분석함으로써 인류의 건강, 나아가 인간의 삶의 질 향상에 크게 이바지할 것으로 기대된다. 이는 학문적 breakthrough일 뿐 아니라 산업적, 특히 의학 분야 산업에 지대한 영향을 주게 될 것이다.
