Total joint replacement is one of the most commonly performed and successful operations in the field of orthopedics as defined by clinical outcomes and implant survivorship. Robot assisted knee arthroplasty is proven to have better surgical outcomes (increased prosthetic alignment accuracy) with precise pre-operative prosthetic cavity planning and accurate milling execution. Incorrect alignment can lead to abnormal wear, premature mechanical loosening, and patella femoral problems.
Registration, one of the key aspects of robotic arthroplasty, is a process of transforming the medical images (milling plan) into the robot’s reference coordinates in the operating room. Patient’s coordinate information (surface data) is extracted using a digitizer, a probe-like sensor, for arthroplasty robotic systems. Drawbacks of digitizer include increased operation time for digitizing sufficient number of points (including calibration), user complications, and addition of source of error (from hand operated digitizer).
In this paper, a registration method using a 3D laser scanner is proposed and experimentally verified. A test-bed is designed to resemble patient’s lower limb in actual TKA. 3D model of the femur is generated using axial slice images of CT scan. ICP algorithm is utilized for registration of CT 3D model and the surface data. Laser Scan Detectable (LSD)-Shape pin is designed for registration during the milling and to verify the experimental results.
Four local regions of distal femur (separated by anatomical landmarks) are evaluated to explore recommended scanning region (RSR). Alignments in coronal and sagittal axis are used to access the clinical usability. An alignment error within ±3° and 2mm in translation (which are considered to prolong prosthetic life) is used as the threshold criteria for clinical use in this study.
Experiments results showed that the local region I (including distal point, medial condylar, and lateral condylar) and II (including anterior patella surface, anterior lateral surface, and anterior medial surface) are sufficient for clinical use. Local region II was found to be the most recommended scanning region (RSR). However, local region I is recommended for Minimally Invasive Surgery. Registration test during the milling, using LSD-Shape pin, was found to be clinically acceptable.
In conclusion, local distal femur regions of the distal femur with high curvature anatomy including condylar regions and anterior surface (excluding the femoral stem area) were given the best results for clinical use. In case of registration during or after milling of the RSR (possible scenario during the actual TKA), a LSD-Shape pin can be used for registration for faster and simple registration using the 3D laser scanner.
본 논문은 로봇을 이용한 인공 무릎관절 수술 (Knee Arthroplasty/Replacement) 시스템의 레지스트레이션(Registration)에 3D 레이져 스캐너를 이용하는 방법론이다. 레지스트레이션 과정은, 로봇의 상대좌표와 실제 수술시 환자의 상대좌표, 그리고 수술 전 CT 를 통하여 얻어지는 무릎 임플란트의 삽입부분이 플랜되어있는 이미지, 이 세개의 좌표를 연결하는 것이다. 대표적인 무릎 인공관절 수술 로봇으로는 ROBODOC, CASPAR, Acrobot, 등이 있는데, 모두 이 레지스트레이션이 이루어진 후 수술이 가능하다. 특히, 현재의 로봇 시스템의 경우, digitizer (point probe) 이라는 기구를 써서 수술시 환자의 좌표를 한점 한점 뽑아낸다. 그러나 digitizer 의 경우, 한번에 한 좌표점밖에 획득할 수 없다. 그리하여 정확도를 높이기 위한 많은 좌표점들을 획득하기에 있어, 전체적인 수술시간이 길어지고, 또한 집도의에게 지루함과 불편함을 초래할 수 있다. 더불어, digitizer 의 경우, 손으로 직접 좌표를 획득하는 방식은 에러의 소스가 되어, 실제로 필요한 좌표보다 더욱 많은 좌표를 획득하여야 한다. 반면, 3D 레이져 스캐는 한번의 스캔으로 많은 좌표점들을 획득할 수 있다. 특히, 사용이 편하고 스캔 과정이 자동이여서 에러 소스를 줄일 수 있다. 본 논문의 최종목표는 3D 레이져 스캐너를 이용하여 인공 무릎관절 로봇 시스템의 레지스트레이션에 사용 가능성 증명이다. 이를 위하여, 모델 뼈의 CT 로 CT 3D Model 을 만들고, 같은 모델뼈의 레이져 스캔된 Surface Data 를 ICP(Iterative Closest Point) 매칭 방법론을 이용하여 레지스트레이션 시킨다. 3D 레이져 스캐너 사용 시, 인공 무릎관절 수술에 가장 적합한 스캔 부위를 4 local region 으로 나누어 실험을 하였다. 4 부분의 local region은 distal femur(정강이뼈)의 특정한 돌출부위(anatomical landmarks)를 기준으로 나누어 실험하였다. 그중, local region 1 과 2 부분이 (distal point, medial condylar, and lateral condylar 그리고 anterior patella surface, anterior lateral surface, and anterior medial surface) 로봇을 이용한 인공 무릎관절 수술에 적합하는 결과를 얻었다. LSD (Laser Scan Detectable)-Shape pin 을 이용하여, 레지스트레이션에 사용되는 부위가 수술중 절삭이 되었더라도, 다시 레지스트레이션을 할 수 있다. 특히, MIS (Minimally Invasive Surgery) TKA 에도 위 두 local region 이 사용될수 있다는 결과를 얻었다.
