In response to vascular stress, vascular smooth muscle cells (VSMCs) specialized to contract can be induced to lose their contractile function and acquire synthetic functions including inflammation, proliferation and migration. In part I and II, I will describe the possible mechanism by which VSMCs increase the synthetic activity in response to treatment with cytokines and growth factor.
Tumor necrosis factor-$\alpha$ (TNF-$\alpha$) and TNF-related apoptosis-inducing ligand (TRAIL) are best known for their selective cytotoxicity of transformed tumor cells. Recent studies revealed that most non-transformed primary cells and several cancer cell lines are not only resistant to death receptor-induced apoptosis, but also subject to pro-inflammatory responses in a NF-$\kappaB$-dependent manner. While involvement of these cytokines in a variety of vascular disorders has been proposed, the exact molecular mechanisms are unclear. In the present study, we aimed to delineate the role of TNF-$\alpha$ and TRAIL in VSMC proliferation. We also sought possible molecular mechanisms to identify potential targets for the prevention and treatment of post-angioplastic restenosis and atherosclerosis. We demonstrated that treatment with TNF-$\alpha$ and TRAIL increased intercellular adhesion molecule-1 expression in VSMCs via differential activation of PKC$\delta$ and subsequent generation of intracellular reactive oxygen species and NF-$\kappaB$ activation. Following detailed analysis using various PKC$\delta$ mutants, we determined that PKC$\delta$activation was mediated by caspase-dependent proteolysis upon TRAIL ligation, while phosphorylation was the main mechanism responsible for TNF-$\alpha$-induced signals. The pivotal role of PKC$\delta$ in vascular pathology was further confirmed by its protective effect on posttraumatic vascular remodeling after in vivo knockdown. These results collectively indicate the pivotal role of PKC$\delta$ in TNF-$\alpha$- and TRAIL-induced vascular inflammation through divergent activation mechanisms.
A reduction in the level of thioredoxin 1 (Trx1) has been proposed as a possible mechanism for the tumor-specific growth arrest caused by inhibition of histone deacetylases (HDACs). In Part II, I investigated the effect of trichostatin A (TSA), a potent HDAC inhibitor, on the proliferation and migration of VSMCs, and I examined the role of reduced Trx1 levels in this effect. TSA treatment time-dependently decreased Trx1 expression in rat VSMCs at both the mRNA and protein levels. It also enhanced platelet-derived growth factor (PDGF)-induced proliferation and migration of the VSMCs. By potentiating Akt phosphorylation, the siRNA-induced downregulation of Trx1 also enhanced VSMC proliferation and migration in response to PDGF or serum treatment. Consistent with these results, TSA administration increased neointimal thickening in a murine model of post-angioplastic restenosis. These data demonstrate that TSA enhances vascular proliferative activity by downregulating Trx1, thus activating an Akt-dependent pathway. Results of part II indicate that, in addition to its apoptotic effects in tumor cells, the downregulation of Trx1 has a proliferative role in primary VSMCs.
The differentiated VSMC exhibits a contractile phenotype in normal vessels and they undergo rapid and reversible phenotypic change in response to vascular injury. Despite bidirectional phenotypic modulation of VSMC phenotype is an important concept for the pathogenesis of various vascular diseases, there remains no cell culture system whereby VSMC differentiate, which make difficult to demonstrate the molecular mechanism for gain or loss of contractile function in VSMC. To induce differentiated phenotype of VSMC, I modulated the cell morphology and organization using micropatterned-groove substrate, based on the concept that the cell morphology and organization may regulate VSMC differentiation and subsequent function. The physiological properties of VSMCs on the grooved surface provide direct demonstration that cultured adult VSMC convert state from a synthetic to a contractile state, which is explained by increased myocardin expression and functional response to vasoactive hormone, norepinephrine. Elongated cells on the grooved substrate also showed the higher expression mesenchymal markers and an increase in directional motility compared with cells on the non patterned substrate, indicating elongated cells on the grooved substrate exert clear mesenchymal phenotype. Dedifferentiation response to culture condition in flat substrate seems to allow cells to retain the secretory phenotype like epithelial cell, which is supported by the observation that many of epithelial markers are upregulated in the cells on the flat substrate. The fact that VSMC cultured on micropatterned substrate can convert between a contractile and synthetic phenotype indicates that they may be a valuable model for this aspect of vascular disease. Therefore, by genomic approach using this $\It{in vitro}$ culture model, I selected the most differentially expressed genes between the differentiated and dedifferentiated states. To increase the physiological relevance and the specificity of target gene identification, microarray gene expression data integrated with proteomic data which were obtained from the animal model of arterial remodeling. 38 proteomic targets in rat ligation model were identified by time-dependent expression analysis in arterial tissue samples. I finally isolated three genes including OLR1, PCNA and ALDOC as potential therapeutic target genes by combining genomic-proteomic approaches. First target candidate that I focused on was OLR1 since its regulation and function were still unknown in VSMCs. I further demonstrated the involvement of OLR1 in neointima formation in response to vascular injury by the results that the neointimal growth in $\It{in vivo}$ ligation model was decreased after transfection of OLR 1 siRNA, consistent with $\It{in vitro}$ assay.
혈관민무늬근세포의 형질 (phenotype) 변화는 혈관질환의 진행정도를 반영하는 중요한 특징이라고 할 수 있기 때문에 혈관질환을 억제하기 위해서 민무늬근세포의 형질을 조절하고자 하는 많은 시도가 있었다. 발생과정에서는 주로 분화 (differentiation)를 촉진하는 다양한 사이토카인 자극이 알려져 있는 반면 혈관질환에서는 세포의 증식이나 이동의 증가와 같은 미분화 (dedifferentiation) 된 형질을 유도하는 사이토카인, 성장인자, 활성산소종과 같은 자극이 보고되어 있고 이들의 작용을 억제하기 위한 다양한 억제제 또한 계속적으로 스크리닝되거나 그 기전이 연구되고 있다. 이 가운데 활성산소종의 경우, 다양한 자극에 의해 생성되어 세포내 구조물을 산화시킴으로써 직접적인 물리적 손상을 가하기도 하지만 2차신호전달자로 신호전달과정의 여러 물질의 활성을 조절하면서 심혈관계 질환을 포함한 다양한 질병의 진행 과정에 중요하게 작용하는 것으로 잘 알려져 있다. 따라서 이러한 활성산소종이 어떠한 기전으로 증가하며 최종적으로 활성산소종이 작용하는 표적단백질을 밝히는 것은 질병의 기전을 연구하고 치료법을 개발하는데 중요하다고 할 수 있다.
첫번째 part에서 활성산소종을 생성함으로써 세포의 형질을 조절하기 위한 자극으로 사용된 TNF superfamily는 전염증성 사이토카인으로 세포의 사멸, 성장을 조절하는 것으로 잘 알려져 있으며, 다양한 혈관 질환시 이들 사이토카인의 비정상적 조절 가능성이 제시되고 있지만 정확한 분자적 기전은 아직 규명되어야 할 부분으로 남아 있다. 그러므로 본 연구에서는 TNF superfamily에 속하는 두가지 사이토카인에 의해 생성된 활성산소종 신호전달 과정을 연구하고자 하였다. 활성산소종 생성을 유도하는 TRAIL과 TNF-$\alpha$ 처리시 세포내 접착분자 ICAM-1이 발현되는 것을 관찰하였고 이 과정에서 redox regulation 되는 것으로 알려진 전사인자인 NF-$\kappaB$가 활성화되는 것을 확인하였다. 또한 활성산소종의 증가에는 TRAIL과 TNF-$\alpha$ 신호전달과정에서 서로 다른 기전을 통해 활성화 되는 PKC$\delta$가 작용하는 것을 증명하였다. 다양한 활성화 기전을 가진 PKC$\delta$는 TRAIL 신호전달과정에서는 세포내 protease인 caspase에 의해 잘림으로써 활성화 되었으며 TNF 처리시에는 PKC$\delta$ 활성을 조절하는 타이로신 잔기가 인산화되는 것을 관찰하였다. 활성화된 PKC$\delta$는 세포내 활성산소종 생성 시스템인 NADPH oxidase를 활성화시키는 것으로 알려져 있는데 NADPH oxidase subunit인 Nox4의 경우 TRAIL과 TNF 두 신호전달 과정에서 NF-$\kappaB$의 활성화에 의한 ICAM-1의 발현에 영향을 주는 것으로 것으로 확인된 반면 Nox1의 경우는 TNF 신호전달에만 관여하는 것을 확인하였다. Caspase를 활성화시킬 수 있는 TRAIL에 의해서는 세포질에서 PKC$\delta$가 잘릴 것으로 예상되므로 소포체나 소액포 (vesicle)에 존재하는 것으로 알려진 Nox isoform인 Nox4를 활성화시키게 된 것으로 보이며, 반면 TNF-$\alpha$에 의해서는 PKC$\delta$의 인산화가 유도되는 것으로 보아 세포막 가까이에서 diacylglycerol에 의해 활성화된 PKC$\delta$가 Nox4 뿐 만 아니라 세포막에 존재하는 Nox1을 활성화시킨 것으로 보인다.
두번째 part에서는 민무늬근세포의 미분화를 나타내는 대표적 특징인 증식과 이동을 증가시키는 기전으로 활성산소종 생성을 유도하는 histone deacetylase inhibitor인 trichostatinA (TSA) 작용 검증하였다. TSA의 경우 다양한 암세포에서 마이토콘드리아를 통해 다량의 활성산소종을 생성함으로써 암세포의 사멸을 유도하는 기전이 알려져 있었지만 in vivo 재협착 모델에서 오히려 혈관의 협착을 증가시켰고 이러한 증식유도 기능은 민무늬근세포를 이용한 in vitro 실험에서 세포의 성장과 이동을 증가시킴을 통해 재확인되었다. 민무늬근세포에서는 TSA에 의한 활성산소종이 증식에 관여함을 밝히기 위해 먼저 TSA에 의한 활성산소종의 증가를 측정하였으며 증가기전으로는 활성산소종 제거에 관여하는 항산화제인 thioredoxin 1의 감소가 작용하는 것을 확인하였다. 또한 세포의 증식에 중요한 Akt가 활성산소종의 표적 단백질이 될 것으로 예상하여 이를 검증하였고 또한 thioredoxin 1의 knockdown시 Akt의 활성이 증가한 것으로 증식과 이동의 증가기전을 설명할 수 있었다.
민무늬근세포의 형질이 분화 (differentiation)-미분화 (dedifferentiation) 사이에서 양방향으로 조절되는 것은 이미 잘 알려진 개념이지만 민무늬근세포는 in vitro 개대배양시 자발적으로 미분화상태로 진행되기 때문에 분화상태를 유지할 수 있는 배양방법의 부재는 정상조직에서 분화된 상태를 유지하거나 질병에서 잃게 되는 기전을 연구하기 어렵게 한다. 따라서 민무늬근세포의 분화를 유지할 수 있는 in vitro 배양모델이 확립되면 이러한 연구에 유용하게 사용될 수 있을 것으로 기대하였다. 세포의 모양과 organization이 세포의 분화정도나 기능에 영향을 미친다는 기존의 개념에 근거해서 세포모양의 조절을 통해 분화를 유도하고자 마이크로패터닝 방법을 사용하였다. 정상혈관에서 세포는 혈관둘레를 둘러싸서 길쭉하게 배열하기 때문에 이와 유사한 형태를 유도하기 위하여 groove 패턴을 이용하였는데 이는 실제 혈관에서 fiber 형태의 extracellular matrix 구조물이 가진 topography와도 유사한 것을 알 수 있었다. 세번째 part에서는 이러한 groove 패턴을 이용해 민무늬근세포의 형질을 조절하기 위한 연구를 진행하였다. Groove 패턴 위에 세포를 배양하였을 경우 정상혈관조직의 민무늬근세포와 유사한 길쭉한 모양이 유도되었을 뿐 만 아니라 western blot 상에서 확인한 결과 감소되었던 smooth muscle (SM)-marker 중 일부의 발현을 다시 증가시켰고 수축을 유도하는 vasoactive hormone에 대한 반응성을 증가시키는 것을 알 수 있었다. 이러한 결과를 통해 볼 때 groove 패턴에 의한 세포배양은 일부 민무늬근세포의 재분화를 유도하는 것으로 사료된다. 또한 groove 패턴 위에서 배양된 민무늬근세포의 증식을 유도하였을 경우, 패턴이 없는 표면에서 배양된 세포보다 증식은 감소한 반면 이동은 더욱 증가하는 것을 확인하였다. 이는 기존에 잘 알려진 개념인 epithelial mesenchymal transition 과정에서 유도되는 mesenchymal cell의 특징과 유사하며 이는 cDNA microarray data에서 mesenchymal marker가 증가되는 것을 통해 확인할 수 있었다. 뿐 만 아니라 groove 패턴에 의한 SM-marker의 증가를 통해 재분화의 가능성도 다시 한번 확인할 수 있었다. 이상의 cDNA microarray 결과를 통해 패턴이 없는 substrate와 비교했을 때 groove 패턴이 있는 substrate를 이용한 배양시 유도되는 전반적인 genetic reprograming을 확인하였고 두 substrate들 상에서 미분화를 유도하는 물질들로 자극하였을 경우 변화하는 유전자를 찾기 위해 microarray 분석을 수행하였다. 변화율이 큰 상위 200 여 개의 유전자를 사용해 hierarchycal clustering한 결과 분화의 intermediate 상태로 추정되는 조건에서 groove 패턴이 있는 경우와 없는 경우 발현되는 유전자의 차이가 큰 것을 알 수 있었다. 본 연구에 의하면 groove 패턴이 형질의 변화를 유도하는 중요한 요인이기 때문에 여기서 차이가 나는 유전자들을 분리하고자 하였고 세가지 방법을 사용해 분리한 결과 총 1,443개의 유전자를 얻을 수 있었다. 여기에는 기존에 알려진 심혈관계 질환의 biomarker들이 의미있게 포함되어 있었는데 이 가운데 심혈관계 특이적인 표적 유전자 선택하기 위해 proteomics data와 통합하여 분석하였다. In vivo 모델에서 proteomics 를 통해 재협착 진행의 각 단계에서 의미있게 변하는 38개의 표적단백질을 분리하였는데 이 가운데 OLR1, PCNA, ALDOC 세 개의 유전자가 microarray data 분석을 통해 얻어진 유전자에도 포함되어 있는 것을 확인하였다. 최종 분리된 세 개의 유전자는 therapeutic potential이 높은 표적 단백질로 기대되며 이 가운데 재협착 과정 중 민무늬근세포에서 그 기능이 잘 알려져 있지 않은 OLR1의 기능에 초점을 맞추어 그 중요성을 확인하였다. 이상의 part3에서의 결과를 통해 groove패턴을 이용한 민무늬근세포의 배양은 세포의 분화를 유도함으로써 심혈관계 질환 연구에 유용한 모델이 될 것으로 기대하였고 실제 심혈관계 질환의 therapeutic target을 찾는데 적용할 수 있음을 검증하였다.
