The concerns about the reasons for continual rapid increases in health expenditures have driven both government and consumers to seek ways to encourage the health services sectors to be more efficient. The economic aspects of health have widely accepted and contributed to these sectors since the late 1960s.
This thesis deals with the three issues on an efficiency in the production of infant's and child's health and the allocation of health resources. With regard to the production of child's health, the first issue is how the mother's education influences the efficiency of production of child's health through her decision about the allocation of her time, her selection of type and quality of nutrition, home environment, health care and her choice of the production method of the child's health. The second one in terms of the production of infant's health is that comprehensive behavioral approach to improve the infant's health is more cost-effective than the traditional clinical approach. The last one related to the allocation of the health resources is to investigate the effects of medical student training program on the cost and productivity of health centers which treat primary care in ambulatory settings.
Major contributions of this thesis can be summarized as follows: first, we make an attempt to do an integrated study of the allocation of time within the framework of the household production of health. As far as we know, no similar studies are conducted about middle income countries.
Second, when we consider that the level of mother's education plays a decisive role in determining her health behaviour, the investment in women's education can give a policy maker the insight to be likely to yield a return commensurate with or greater than the investment in men's education.
Third, we reexamine the effectiveness of traditional clinical prenatal care and propose a new more promising approach which is designed to demonstrate a more comprehensive integrated one for improving the production of birth outcome measured by infant's health.
Finally, for the efficient allocation of the health resources, we try to analyze the effects of medical student training program on cost and productivity in primary care, not in secondary and tertiary care, in health center whose clienteles are mostly from the low-income population group and where the physicians are employed on a salary basis mostly financed by government.
1960년대 말 70년대에 들어서면서 인간 행위에 관한 경제학적 접근이 광범위하게 시도되었다. 보건/의료 서비스 부분에서의 경제적 측면에 관한 연구도 폭 넓게 진행되면서 이후 30년간 OECD 국가를 선두로 각 나라의 보건/후생 향상에 많은 기여를 하고 있다. 이는 종전에 의료 서비스 일변도의 접근 방법에서 보다 광범위하고 다양한 시각을 제공해 주는 것은 물론 기존의 접근법과의 조화를 통하여 새로운 분야로서의 활로를 개척, 뿌리를 내리고 있다고 할 수 있다.
보건/의료 서비스에 관한 경제적 해석은 가계는 시간, 기술 및 지식 등의 요소들이 투입된 생산과정에 의해서 효용을 창출하는 상품을 생산하는 생산자인 동시에 소비자로 파악된다는 가계 생산 이론에서 출발하여 인간 자본 형성에 주요 투자원으로 기여함에서 비롯된다. 따라서 경제재로서 건강의 생산에 사용되는 투입 요소를 정의함에 있어 단순히 시장 재화와 서비스에 국한하지 않고 생산 과정에 소요되는 시간 요소와 소비자 자신에 의해서 제공된 투입 요소까지도 고려해야 한다는 점에서 기존의 소비자 행위 이론의 재형성이라 할 수 있다.
이러한 접근법에 기초하여 본 논문의 연구 목적은 다음과 같다.
첫째, 아이의 건강의 효율적 생산을 위해서 어머니의 교육 수준이 어떻게 자신의 시간 분배와 건강 생산 기술의 선정 및 기타 주요 투입 변수에 영향을 미치는 가를 파악함에 있다.
둘째, 건강한 아이의 출산을 위해 기존의 단편적 의료 서비스의 접근법과는 달리 이를 포함한 다차원의 행위 지향적 종합적 접근 방법의 비용-효과적 우수성을 검증함에 있다.
마지막으로, 2, 3차 의료 기관에 비해 1차 의료 기관에서의 의료 교육의 중요성을 확인하여 한정된 의료 자원의 효율적 배분을 모색하고자 한다.
인간 자본의 확대를 위한 투자 방안으로서 건강에 대한 투자는 궁극적으로 가계에서 발생되어 지며 이의 핵심적 역할을 수행하는 부모의 행위적 요소가 가장 의미 있는 투입 요소가 될 수 있다고 본다. 더구나 부모와 자녀의 이러한 관계로 축적된 인간 자본은 세대간에 이전됨으로써 그 중요성을 더한다고 볼 수 있다.
본 연구는 한국의 가계 자료를 통해 가계 생산의 틀 속에서 건강 생산과 관련된 여러 투입 요소와 함께 시간 배분의 중요성을 검증한 통합적 연구가 이뤄졌다는 점과 건강 증진에 대한 의료 서비스 일변도의 접근법에서 행위적 접근법의 효용 가치를 분석하였다는 점 및 의료 자원의 효율적 배분의 문제를 실증적 자료를 통하여 분석하였다는 점등에서 의의를 갖는다고 할 수 있다.
