This study focuses on the experimental investigation of the shear bonding strength in the presence of normal stress of the cement-bone interface. For this study, special compressive and shear ‘biaxial’ jig was developed. The cement-bone interface is one of the most important factors in the aseptic failure. The experiments were performed with the small cement-bone specimens to verify the shear bonding strength in the interface. The author followed contemporary hip replacement surgery with a swine femur, prepared specimens by cutting into 10mm ×5mm×5mm (2.5mm: cement + 2.5mm: bone) specimens with low speed diamond cutter and tested it using INSTRON servo microforce system. The results showed that the cement-bone interface shear bonding strength increased with the increase of the normal stress based on the Hoffman’s multi-axial criteria diagram. From the regression equation, it is estimated that shear bonding strength increases 1.7 times (142%) as much as 1MPa incensement of normal stress. It is concluded that normal stress to the interface can decrease the shear failure.
인공고관절 전치환술은 대퇴 경부 골절이나 대퇴 골두의 괴사, 함몰, 관절 연골의 퇴행성 변화가 심할 경우 시행되고 있는 수술로써, 썩거나 파괴된 관절, 골절된 대퇴 골두를 제거하고 대퇴부에 스템이라는 인공 삽입물을 삽입하고, 비구부에는 금속으로 된 컵과 스템과 비구부를 연결하는 볼을 넣는 방법이다. 이는 매년 세계적으로 백만 건 정도 시술 되고 있는 보편화된 수술이지만, 스템이 헐거워지는 현상등으로 인해 이중 10 퍼센트 정도는 재수술을 하고 있는 실정이다.
인공고관절 전치환술에 의한 문제점을 해결하고 장기간의 임상결과를 개선하기 위하여 스템의 디자인에 관한 많은 연구가 시행되어 현재는 시멘트 사용식과 시멘트 비상용식의 방법으로 발전되었다. 특히, 시멘트를 사용하는 방식에 있어서 스템 표면을 매끄럽게 처리하는 방식과 거칠게 처리하는 방식에 대해서 논란이 제기 되고 있다. 본 연구에서는 스템 표면을 매끄럽게 처리한 경우 발생하는 압축응력이 시멘트와 뼈에 미치는 영향에 대해서 실험을 통하여 고찰하였다. 시멘트와 뼈의 경계면은 전단응력이 발생할 때, 둘 사이의 상대운동에 의해서 뼈에 막이 형성되고 상대운동을 증가시켜서 결과적으로는 파괴현상을 일으키게 된다. 그러나, 만약 시멘트와 뼈에 압축응력이 작용한다면 경계면에 전단응력을 줄여줄 뿐만 아니라, 둘 사이의 상대운동도 감소시켜서 파괴 현상을 줄여줄 것으로 예상된다. 따라서, 본 연구에서는 시멘트와 뼈로 이루어진 시편에 대해서 압축력과 전단력이 동시에 작용하는 상황에서의 전단강도를 측정함으로써 시멘트와 뼈 경계면의 압축응력에 대한 효과를 검증하였다.
