Liver is known as an immuno-tolerant organ. Regulatory T (TREG) cells are thought to be involved in this immune tolerance, but few immunologic studies have been conducted in depth. In this study, I obtained liver perfusate from healthy donors during liver transplantation and isolated mononuclear cells. I performed phenotypic and functional immunologic analyses of TREG cells in the liver and found them to be phenotypically more active and functionally more suppressive compared to peripheral blood (PB). And while PD-1 has been well studied in recent years as anti-PD-1 therapies have been actively utilized in cancer treatment, its role in TREG cells is not yet well understood. I characterized TREG cells in the liver based on their expression of PD-1 and performed phenotypic, transcriptomic, and functional analyses of PD-1+ and PD-1- TREG cells and identified unique characteristics of PD-1+ TREG cells in the liver. The results confirmed that PD-1+ TREG cells are phenotypically and transcriptomically more activated and functionally more suppressive than PD-1- Treg cells. I also found that PD-1+ TREG cells in the liver highly express tissue resident markers, unlike PD-1+ TREG cells in the PB, and possess a unique T cell receptor (TCR) repertoire. And when anti-PD-1 treatment was given to TREG cells in the liver, their function was further enhanced. Finally, an increased proportion of PD-1+ cells was observed in TREG cells in the liver in patients with liver cirrhosis. In summary, this study furthered our understanding of TREG cells in the liver and provided a deeper immunologic characterization of PD-1+ TREG cells in particular.
간은 면역관용적인 장기로 알려져 있다. 이러한 면역관용성에 조절 T 세포가 관여할 것으로 생각되나 이에 대해 면역학적으로 깊게 수행된 연구는 적다. 이에 본 연구에서는 간이식 중 건강한 기증자에서 간 관류액을 얻어 단핵구 세포를 분리하였다. 이를 통해 간 내 조절 T 세포에 대해 표현형적, 기능적인 면역학적 분석을 시행하였고, 말초혈액에 비해 간 내 조절 T 세포가 표현형적으로 더 활성화되어 있고 기능적으로 더 억제적임을 확인하였다. 그리고 최근 항 PD-1 치료가 암 치료에서 활발하게 이용되고 있어 PD-1에 대한 연구가 많이 이루어졌으나 조절 T 세포에서의 PD-1의 역할은 아직 정립되지 않았다. 이에 간 내 조절 T 세포를 PD-1의 발현에 따라 구분하여 PD-1 양성 조절 T 세포와 PD-1 음성 조절 T 세포의 표현형적, 유전체적 그리고 기능적인 분석을 시행하였고 간 내 PD-1 양성 조절 T 세포의 독특한 특성에 대해 함께 확인하였다. 그 결과 PD-1 양성 조절 T 세포는 PD-1 음성 조절 T 세포에 비해 표현형적, 유전체적으로 더 활성화되어 있으며 기능적으로 더 억제적임을 확인하였다. 또한 간 내 PD-1 양성 조절 T 세포는 말초혈액의 PD-1 양성 조절 T 세포와 다르게 조직 상주 마커를 높게 발현하며 독특한 T 세포 수용체 레퍼토리를 갖는다는 것을 확인하였다. 그리고 간 내 조절 T 세포에 항 PD-1 치료를 시행했을 때 그 기능이 더 강화되었다. 마지막으로 간경변증 상황에서 간 내 조절 T 세포에서 PD-1을 발현하는 세포의 비율이 증가하는 것이 관찰이 되었다. 본 연구를 통하여 간 내 조절 T 세포에 대한 이해가 높아졌으며 특히 PD-1을 발현하는 조절 T 세포의 심도 깊은 면역학적 특성을 확인할 수 있었다.
