Most stroke patients with upper limb paralysis experience a loss of hand motor function and have difficulty performing activities of daily living. Functional electrical stimulation therapy is a treatment that generates functional movement by applying electrical stimulation to muscles that have lost their motor function. Electrical stimulation devices that are lightweight, small, and inexpensive are effective in improving hand motor function and have high accessibility. However, most electrical stimulation devices cause muscle fatigue by continuously stimulating the flexor muscles of the stroke patient’s bent hand to extend it. In addition, to deliver electrical stimulation, additional manipulation beyond functional movement is required, resulting in asynchronous electrical stimulation that does not match the user’s intention. This study developed a hand rehabilitation glove for stroke patients that overcomes the limitations of functional electrical stimulation therapy while maintaining its benefits. The hand rehabilitation glove uses a hybrid actuation method that extends the fingers through the elasticity of a silicone-based orthosis and bends the fingers through electrical stimulation. In addition, a grasp-assist control strategy is established that recognizes finger flexion, extension intention through a strain sensor embedded in the silicone and determines electrical stimulation. Task-oriented rehabilitation training designed for stroke patients demonstrates that active object grasping assistance based on intention recognition is possible using the hand rehabilitation glove, which improves hand motor function.
상지 마비에 의한 운동 장애를 겪는 뇌졸중 환자 대부분은 손 운동 능력의 상실을 겪으며 일상생활을 수행하는 데 있어 제약을 가진다. 기능적 전기 자극치료는 운동 능력이 상실된 근육에 전기 자극을 가해 수축을 유발함으로써 기능적인 움직임을 생성하는 치료법이다. 가벼운 무게, 작은 부피, 저렴한 가격의 전기 자극 장치는 손 운동 기능 개선 효과와 더불어 높은 접근성을 가진다. 하지만, 대부분의 전기 자극 장치는 뇌졸중 환자의 굽힌 손을 펴기 위한 신전근에 지속적인 전기 자극을 요해 근피로를 유발한다. 또한, 전기 자극 전달을 위해 기능적인 움직임 이외의 추가적인 조작을 요하기 때문에 사용자의 의도와 비동기화된 전기 자극을 생성한다. 본 연구는 기능적 전기 자극치료의 이점을 유지하며 앞선 한계점들을 극복할 수 있는 뇌졸중 환자를 위한 손 재활 장갑을 개발하였다. 실리콘 기반 보조기의 탄성력으로 손가락을 펴고, 전기 자극을 통해 손가락을 굽히는 하이브리드 구동방식을 설계하였다. 또한, 실리콘에 내장된 스트레인 센서를 통해 손가락 굴곡, 신전 의도를 인식하여 전기 자극 생성을 결정하는 파지 보조 제어 전략을 수립하였다. 뇌졸중 환자를 대상으로 설계한 작업 재활 훈련은 손 재활 장갑을 통해 향상된 손 운동 기능을 바탕으로 의도 인식에 기반한 능동적인 물체 파지 보조가 가능함을 보여준다.
