Although remarkable advances have been made in the treatment of malignant tumors over the past few decades, the treatment of glioblastoma has not shown great progress. Immune checkpoint inhibitors (ICIs), including anti-programmed cell death protein-1 (anti-PD-1), have shown excellent effects against various tumors, including melanoma, but have failed to improve on the effects of existing treatments in several clinical trials in glioblastoma. Furthermore, the gut microbiota was revealed to be closely related to the anti-tumor immune response and enhance the effect of ICIs. This study found that the survival rate of glioblastoma was increased through a high-glucose diet (HGD) and showed that the change in the gut microbiota induced by the HGD caused this difference in the survival rate of glioblastoma. In addition, phenotypic analysis of immune cells in the glioblastoma tumor microenvironment showed that the HGD increased the immune response of cytotoxic CD8+ T cells. Moreover, an experiment performed to compare survival rates confirmed that this increase in the anti-tumor immune response mediated by the HGD increased the effect of the ICI. Furthermore, we identified specific gut microbiota species that are increased by the HGD and found that these species play an important role in enhancing the anti-tumor immune response. Consequently, this study reveals that an HGD induces changes in the gut microbiota, and these changes could increase the survival rate of glioblastoma by upregulation of anti-tumor immunity.
지난 수십년 동안 악성 종양의 치료에 있어서 괄목할 만한 진보가 있어 왔음에도 불구하고, 교모세포종의 치료는 큰 발전을 이루지 못하였다. 특히, anti PD-1을 비롯한 면역 관문 억제제는 흑색종을 비롯한 다양한 종양에서 뛰어난 효과를 보였으나, 교모세포종에서 그 효과를 증명하기 위한 여러 임상 실험에서 기존 치료법에 비해 더 나은 효과를 증명하는데 실패하였다. 이러한 배경에서 최근 장내 미생물이 항 종양 면역반응과 밀접한 연관이 있으며, 이를 통해 면역 관문 억제제의 효과를 증진시키는데 영향을 준다는 것이 밝혀졌다. 본 연구는 고 포도당 식이를 통해 교모세포종에서의 생존율이 증가한다는 것을 발견하였고 장내 미생물의 변화가 이러한 생존율 변화의 원인임을 증명하였다. 또한 교모세포종 내 면역세포의 표현형 분석을 통하여, 고 포도당 식이가 세포독성 T 세포의 면역반응을 상승시킨다는 것을 확인하였으며 결과적으로 이러한 항 종양 면역반응의 상승은 교모세포종에서 나타나지 않았던 면역 관문 억제제의 효과를 상승시키는 것을 생존율 비교 실험을 통하여 확인하였다. 더 나아가, 고 포도당 식이로 인해 증가하는 장내 균주를 특정하고 해당 균주가 항 종양 면역반응을 증진시키는데 중요한 역할을 한다는 것을 발견하였다. 결과적으로 고 포도당 식이는 장내 미생물의 변화를 유도하여 교모세포종의 생존율을 증가시키며 면역 관문 억제제의 적용을 유도할 수 있다는 것을 밝혔다.
