Regulatory T cells (Tregs) exert immunosuppressive functions and hamper antitumor immune responses in the tumor microenvironment. Understanding the heterogeneity of intratumoral Tregs, and how it changes with tumor progression, will provide clues regarding novel target molecules of Treg-directed immunotherapies. Here I characterized the phenotypes and suppressive function of Tregs from tumor tissues of patients with renal cell carcinoma and ovarian cancer using multicolor flow cytometry, RNA sequencing, and in vitro functional assay, and CT26 mouse tumor model. I found that carcinoembryonic antigen-related cell adhesion molecule 1 (CEACAM1) was selectively expressed on intratumoral Tregs, while its expression on peripheral Tregs or other immune cells was low. In consequence, CEACAM1$^+$ intratumoral Tregs accumulated with tumor progression in both RCC patients and mouse tumor model, while the CEACAM1$^–$ subset did not. Notably, I found that CEACAM1 marked intratumoral Tregs that exhibited highly suppressive and activated phenotypes with substantial clonal expansion. CEACAM1$^+$ intratumoral Tregs exhibited increased suppressive capacity compared to CEACAM1– counterpart in both in vitro and in vivo. Depletion of CEACAM1-expressing cells from tumor-infiltrating leukocytes led to selective depletion of Tregs and increased effector functions of CD8$^+$ and CD4$^+$ T cells. Moreover, CEACAM1$^+$ cell depletion further enhanced anti-PD-1–mediated reinvigoration of exhausted CD8$^+$ T cells. These findings demonstrate that CEACAM1 is an optimal target for intratumoral Treg depletion.
조절 T 세포는 면역억제 기능을 하며 종양 미세환경에서 항종양 면역반응을 억제한다. 종양내 조절 T 세포를 표적으로 하는 새로운 항암치료 전략의 표적 단백질을 찾기 위해서는 종양내 조절 T 세포의 이질성을 파악하고 종양의 진행에 따라 그 특성이 어떻게 변화하는지 이해해야 한다. 이에 본 논문에서는 신장암과 난소암 환자의 검체와 CT26 마우스 종양 모델을 이용해 유세포 분석, RNA 시퀀싱 및 생체외 실험을 시행하여 종양내 조절 T 세포의 특성과 기능을 연구했다. 종양내 조절 T 세포에 CEACAM1의 발현이 높았던 반면 말초 혈액의 조절 T 세포나 다른 종양내 면역세포에서는 CEACAM1의 발현이 낮았다. 신장암 환자와 CT26 마우스 종양 모델에서 CEACAM1을 발현하는 종양내 조절 T 세포는 종양이 진행할수록 증가했지만 CEACAM1을 발현하지 않는 종양내 조절 T 세포는 증가하지 않았다. 특히 CEACAM1을 발현하는 종양내 조절 T 세포는 억제능이 강하고 활성화된 표현형을 보였으며 클론성이 높아져 있었다. 그리고 실제로 CEACAM1을 발현하는 조절 T 세포는 그렇지 않은 조절 T 세포에 비해 생체외 및 생체내에서 강한 면역억제능을 보였다. 종양내 면역세포에서 CEACAM1을 발현하는 세포들을 제거했을 때 조절 T 세포가 선택적으로 제거되었으며 CD4 T 세포와 CD8 T 세포의 기능이 높아졌다. 또한, CEACAM1을 발현하는 세포를 제거하면 탈진화된 CD8 T 세포의 기능이 PD-1 항체 치료로 인해서 더욱 높은 폭으로 회복됐다. 결론적으로 본 연구에서는 CEACAM1이 종양내 조절 T 세포를 제거하는 치료 전략의 적절한 표적 단백질이 될 수 있음을 보여주었다.
