In recent decades, many attempts have been made to identify biomarkers and build prediction models for depression, using the resting-state functional connectome. However, most of these have used brain connectivity, based on temporal correlation and small-scale datasets; they are therefore vulnerable to confounding effects. The present study has obtained a whole-brain causal connectome from the brain’s resting-state functional MRI signals. Data drawn from three large-scale datasets (YAD, Young-Age Depression: dataset of 294 participants; EMBARC, Establishing Moderators and Biosignatures of Antidepressant Response in Clinical Care: dataset of 162; and HCP, Human Connectome Project: dataset of 865 subjects) were used to implement a depression-prediction model, based on machine learning and deep learning. A graph neural network-based model capable of learning directed causal-graph properties showed the best performance; its explanatory model suggested relevant brain regions and the connectivity of depression. The framework applied in this study is expected to shed light on the pathophysiology of depression at both the individual and group levels, providing assistance for diagnosis and therapeutic decision-making in clinical practice.
휴지기 뇌 연결성을 이용한 우울증의 생체 지표 탐색과 예측 모델 개발이 시도되어 왔지만 대부분 시간적 상관성에 기반한 뇌연결성을 사용하여 교란변수 에 취약했고 대규모 데이터셋에 적용한 연구는 많지 않았다. 본 연구에서는 세 대규모 데이터셋(청년기 우울증 데이터셋 268명, EMBARC 데이터셋 163명, HCP 데이터셋 865명, 총 1296명)의 기능적 뇌자기공명영상 으로부터 피질 및 피질하 영역을 모두 포함한 전뇌 인과적 연결성 그래프(causal connectome)을 얻어내어 기계학습 및 딥러닝을 이용한 우울증 예측 모델을 구현하였다. 방향성이 있는 인과적 그래프의 특성을 잘 학습할 수 있는 그래프 뉴럴 네트워크 기반 모델이 가장 좋은 성능을 나타내었고, 이에 대한 설명모델 역시 기존의 알려진 우울증의 신경학적 기저 (neural basis) 및 연결성(connectivity)에 어긋나지 않아 타당하고 설명 가능한 예측 모델임을 알 수 있었다. 본 연구에 적용된 프레임워크는 집단 뿐만 아니라 개인 수준에서의 우울증 병태 생리를 이해하고 진단 및 치료적 임상 의사 결정을 돕는데에 유용하게 활용될 수 있으리라 기대한다.
