Endovascular intervention is a minimally invasive and image-guided procedure performed to diagnose and treat diseases in the blood vessels using long and flexible instruments known as catheters and guidewires. There have been a few simulations developed to practice the intervention procedures. Haptic sensation is essential to the simulation that trains how to use the haptic cues during the procedure. This dissertation proposes a haptic interface for the simulation of cardiac catheters. The proposed haptic interface provides both active and passive haptic forces using a hydraulically driven soft actuator. The designed haptic actuator has a torus outer shape with an ellipse-shaped inside chamber which is actuated by hydraulic pressure. Expansion of the chamber can squeeze the inside passage of the catheter to hold the catheter traveling through. This also simultaneously pushes the catheter in the axial direction to render active haptic forces. The size of the catheter passage is adjustable by providing negative pressure to the chamber, allowing various diameters of the medical catheters to be used in the simulation.
The diameter of the catheter passage can be enlarged up to 1.6 times to allow 5 to 7 Fr (1 Fr = 1/3 mm) catheters to pass through with low resistive force. Experiment results show that the proposed haptic actuator can render 0 ~ 2.0 N passive feedback force, and a maximum of 2.0 N of continuous active feedback force, sufficient for the cardiac catheter simulation. The haptic actuator can render a nonlinear force profile of passive force with 0.08 N RMS and 9.94% of L2-norm relative errors for 6 and 7 Fr catheters moving with constant velocity. The haptic actuator renders passive force to a 5 Fr catheter with 0.10 N RMS and 10.51% L2-norm relative errors. The haptic actuator can render passive force with 0.11 N RMS and 11.20% L2-norm relative errors for catheters moving with a variable velocity. The haptic actuator is shown to render a continuous active force profile with 0.10 N RMS and 10.30% L2-norm relative errors for 5, 6, and 7 Fr catheters.
The proposed haptic actuator can provide active and passive forces with sufficient force range and accuracy for catheter simulation. The passive and active force feedback errors are within the human JND (Just Noticeable Difference) of force for 5, 6, and 7 Fr catheters.
혈관중재시술은 가늘고 긴 유연한 시술도구인 카테터와 가이드와이어를 인체에 삽입하여 혈관의 질병을 진단하고 치료하는 최소침습시술이다. 혈관중재시술 훈련을 위한 햅틱 기반의 컴퓨터 시뮬레이션이 개발되었으며, 햅틱 감각은 시술 훈련에 필수 요소이다. 본 논문은 심장 카테터 시술 훈련 시뮬레이션을 위한 햅틱 인터페이스를 제안한다. 제안된 햅틱 인터페이스는 유압으로 구동되는 유연구동기를 사용하여 능동 및 수동 햅틱 피드백을 모두 제공할 수 있다. 설계된 햅틱 구동기는 유압에 의해 작동되는 타원형 챔버가 내장되어 있고, 토러스 모양의 외부 형상을 가지고 있다. 챔버를 확장하면 햅틱 구동기의 통로가 좁혀져 카테터의 움직임에 저항력을 제공하고, 동시에 햅틱 구동기가 변형하여 카테터를 잡고 축 방향으로 밀어내어 능동 햅틱 피드백을 렌더링할 수 있다. 카테터 통로의 크기는 챔버에 음압을 가하여 조정할 수 있으며, 이를 통해 다양한 직경의 의료용 카테터를 시뮬레이션 중에 교체하여 사용할 수 있다.
햅틱 구동기 통로의 직경은 5 - 7 Fr (1 Fr = 1/3 mm)의 카테터가 낮은 저항력으로 통과할 수 있도록 최대 1.6배까지 확장된다. 비선형 힘 프로파일 추정 실험 결과는 제안하는 햅틱 구동기는 0-2.0N의 수동 햅틱 피드백과 최대 2.0N의 연속적인 능동 햅틱 피드백을 렌더링할 수 있다. 일정한 속도로 움직이는 6, 7 Fr 카테터에 대해 햅틱 구동기는 0.08N의 RMS(root-mean-square)와 9.94%의 L2-norm 상대 오차를 가지며, 5Fr 카테터에 대해서는 0.10N의 RMS 및 10.51%의 L2-norm 상대 오차를 갖는다. 변하는 속도로 움직이는 카테터에 대해서는 0.11N의 RMS 및 11.20%의 L2-norm 상대 오차로 수동력을 렌더링할 수 있다.
햅틱 구동기는 5, 6, 7 Fr 카테터에 대해 0.10 N의 RMS 및 10.30% L2-norm 상대 오차로 능동 햅틱 피드백을 렌더링할 수 있다. 제안된 햅틱 구동기는 카테터 시뮬레이션을 위한 충분한 힘 출력 범위와 정확도로 능동 및 수동 햅틱 피드백을 제공할 수 있다. 5, 6, 7 Fr 카테터에 대한 수동 및 능동 햅틱 피드백 오차는 인간의 힘 JND(Just Noticeable Difference) 범위 내에 있다.
