The renal sympathetic denervation is an minimum invasive procedure and an representative and effective treatment to the resistant hypertension, which has no effect on curing hypertension by medicine treatments. The ablation of the nerves, which is on the wall of renal artery, suppresses the acting of the afferent sympathetic nerves and the efferent sympathetic nerves to the kidneys decreasing the blood pressure.
The procedure is to cut the thigh, insert the catheter, which composed of a long wire and electrodes on the end of the wire, to the renal artery, contact the electrodes on the endplace with the wall of the artery and deliver an energy which is supplied by power generator to ablate renal sympathetic nerves on the wall of renal artery. At this time, the sorts of the energy is divided into the form of radiofrequency, ultrasonic and heat and the end of the catheter consists of the electrodes, which deliver the energy, the temperature sensor and the impedance sensor, all of which is connected to the control module outside body by the wires. The RDN procedure usually has more than six regions for ablation to cut off an cylindrical artery perfectly so that it is to be most effective method that several region is to be ablated at the same time by using multi-electrodes, and consequently the time of the procedure is to be minimized and the complications of patients are also to be reduced. However, the wires for transmitting temperature information and impedance information and delivering the energy are increasing unavoidably according to the increase of the number of electrode. Such the increase of wires not only raises the cost for the manufacturing due to the decrease of manufacturing yield but also makes the flexibility of catheter decrease.
To resolve this problems, it is proposed in this paper that application specific integrated chip(ASIC), which replace the existing electrode, and the system which uses catheter that ASIC is embedded in. To communicate with only one data line, the time division multiple access(TDMA) structure is used. Also, temperature sensor, impedance sensor and analog-to-digital-converter are integrated in one chip that replace the existing electrode for whole system operation.
신장신경절제(Renal denervation)술이란 최소 침습적 시술로서 약물치료가 효과가 없는 저항성 고혈압에 대한 대표적이고 효과적인 치료법이다. 신장동맥벽 신경의 절제는 원심적 교감신경과 구심적 교감신경의 신장에 대한 작용을 억제시켜 혈압을 감소시킨다. 이 시술은 허벅지를 통해 전극이 달린 카테터(Catheter)를 신장 동맥까지 삽입하여 신장동맥벽에 존재하는 신경에 에너지를 가해 절제하는 시술이다. 이 때 전극을 통해 신장동맥벽에 가해지는 에너지의 종류는 고주파, 초음파, 열 등의 형태로 나눠지고 카테터의 끝은 에너지를 전달하는 전극과 온도센서, 저항센서로 구성되며 외부의 제어장치와 전선으로 연결된다. 한 시술은 보통 6 부위 이상의 절제 부위를 가지는데, 다중전극을 이용하여 이러한 여러 부위를 동시에 절제하는 것이 시술시간을 최소화하고 환자의 합병증을 줄이기 위한 효과적인 방법이다. 그러나 다중전극을 이용하기 위해서는 온도 정보를 외부로 보내주기 위한 전선과 에너지를 전달하기 위한 전선들의 수가 전극이 증가함에 따라 불가피하게 증가하게 된다. 그것은 카테터의 유연성을 감소시킬 뿐 아니라 제조수율을 감소시켜 제조비용이 증가하게 한다. 이 문제를 해결하기 위해 이 논문에서는 전극을 대체하는 집적회로가 내장된 카테터를 이용한 신장신경절제술 장비가 제안되었다. 하나의 전선으로 여러 전극에 에너지를 전달하고, 제어장치와 전극 대체 칩 사이에 데이터(data)를 주고받기 위해 시간분할다중접속 구조가 이용되었고, 온도센서, 임피던스센서, 아날로그-디지털 변환기가 전극 대체 칩 안에 집적되었다.
