Rising pharmaceutical expenditure is burdening global health care systems and also coincides with the increased spending on pharmaceutical marketing. Therefore, this dissertation proposes several research questions related to the physicians’ prescribing to successfully use limited resources. The first essay investigates how an increase in patient information sharing among doctors impacts health care costs. Specifically, the study explores this impact through two mechanisms - the informative role of patient health conditions and the cross-monitoring role against doctor-driven induced health care demands. The study utilizes a unique policy intervention (a drug utilization review) introduced in 2009 in Korea that enables doctors to share outpatients’ prescription histories. Using difference-in-differences, the study finds that, when patient information is improved, there is a reduction in pharmaceutical spending. This result is especially true for those patients who have relatively weak information-sharing capabilities. Using the data on the amount of antibiotics prescribed for the common cold, the study shows that a cross-monitoring of prescriptions among doctors reduces the amount of unnecessary prescriptions and thus health care spending. The second essay finds empirical evidence that supports the possibility that a physician has different values for different firms. The study measures a physician’s brand-specific future value estimated by two stochastic models, BG/NBD and GG model, which consider physicians’ various prescription history information including frequency, timing, and quantity decisions. Using physician-level panel data for erectile dysfunction drug market where the three drugs (Viagra, Levitra, and Cialis) compete in the category, the result shows that there exists misallocation of detailing, especially for Levitra. This dissertation provides implications to both policymakers and pharmaceutical firms to successfully utilize limited resources in the pharmaceutical market.
약품비의 증가는 전 세계 의료 시스템에 부담을 주고 있으며, 또한 제약 마케팅의 지출 증가에도 영향을 미치고 있다. 이에 본 학위 논문은 제한된 자원을 성공적으로 사용하기 위하여, 의사의 의약품 처방과 관련된 몇 가지 연구 질문을 제안한다. 첫 번째 연구는 의사들 간의 환자 정보 공유가 증가할 때 의료 비용에 미치는 영향을 분석하였다. 이는 특히 환자의 건강 상태에 대한 추가적인 정보를 제공하는 역할과 의사의 유인 수요를 제한하는 교차 모니터링의 역할의 두 가지 메커니즘을 통해 이루어짐을 본 연구를 통해 확인하였다. 본 연구에서는 2009년 한국에 적용된 정책을 통해 의사의 환자 정보 공유 영향을 분석하였다. 해당 정책은 의약품 처방 조제 지원 시스템으로, 의사가 외래 환자의 처방 내역에 대한 정보를 공유할 수 있도록 하였다. 이중 차분 분석 결과, 의사간의 환자 정보 공유의 증가가 약품비를 줄이는 것을 확인하였다. 이러한 약품비 감소는 특히 정보 공유 능력이 부족한 환자 집단에 대해 효과를 보였다. 또한, 의원에서 감기에 처방된 항생제의 양을 의사의 유인 수요의 측정에 사용하여, 의사들 사이의 교차 모니터링 역할이 불필요한 약의 처방을 줄여 의료비를 줄일 수 있음을 밝혔다. 두 번째 연구에서는 실증 분석을 통해 의사가 여러 제약회사에게 서로 다른 가치를 가진다는 것을 확인하였다. 본 연구에서는 의사의 약물 처방 빈도, 처방 시점 및 처방 수량 등의 의사의 처방 행위에 대한 정보를 활용하여 두 가지 확률적 모형을 통해 브랜드 별 의사의 가치를 측정하였다. 세 브랜드가 경쟁하는 발기 부전 치료제 약물에 대한 의사의 처방 데이터를 활용하여 분석한 결과는 의사에 대한 방문 마케팅이 최적으로 이루어지지 않고 있음을 보였다. 본 학위 논문은 두 가지 연구를 통해 정책 입안자와 제약 회사 모두에게 제한된 자원을 활용하는 것에 대한 함의를 제공한다.
