Nonalcoholic fatty liver disease (NAFLD) is increasing in worldwide prevalence, closely tracking the obesity epidemic, but specific pharmaceutical treatments for NAFLD are lacking. Defining the key molecular pathways underlying the pathogenesis of NAFLD is essential for developing new drugs. In this study I demonstrate that inhibition of gut-derived serotonin (GDS) synthesis ameliorates hepatic steatosis through a reduction in liver serotonin receptor 2A (HTR2A) signaling. Local serotonin concentrations in the portal blood, which can directly travel to and affect the liver, are selectively increased by high-fat diet (HFD) feeding in mice. Both gut-specific Tph1 knockout mice and liver-specific Htr2a knockout mice are resistant to HFD-induced hepatic steatosis, without affecting systemic energy homeostasis. Moreover, selective HTR2A antagonist treatment prevents HFD-induced hepatic steatosis. Thus, the gut TPH1-liver HTR2A axis shows promise as a drug target to ameliorate NAFLD with minimal systemic metabolic effects.
비만 인구의 증가에 따라 비알코올성 지방간질환의 유병률은 전세계적으로 증가하고 있지만, 이에 대한 치료제 개발은 전무한 실정이다. 비알코올성 지방간질환 발병기전의 핵심 분자 경로를 규명하는 일은 치료제 개발을 위해 필수적이다. 이에 본 학위논문에서는 장에서 유래하는 세로토닌의 합성을 억제하면, 간세포에 존재하는 세로토닌 수용체 2A 신호전달경로 활성화 억제를 통하여 지방간 발생을 억제할 수 있음을 증명하였다. 생쥐에서 고지방식이를 투여하면, 장에서 간으로 혈액이 이동하는 통로인 간문맥에서 혈중 세로토닌 농도가 선택적으로 증가하는 것을 입증하였다. 또한 장세포 특이적 Tph1 결손 생쥐 모델과 간세포 특이적 Htr2a 결손 생쥐 모델에서 전신적인 에너지 대사의 변화 없이 고지방식이에 의해 유도되는 지방간이 발생하지 않음을 확인하였다. 그리고 고지방식이에 의해 유도된 지방간 생쥐모델에 선택적 세로토닌 수용체 2A 길항제를 투여하면, 지방간이 호전됨을 확인하였다. 이러한 실험 결과들을 종합하여 본 연구는 장의 TPH1-간의 HTR2A 축을 전신 대사에 미미한 영향을 미치면서 비알코올성 지방간질환을 개선시키는 약물 표적으로 사용할 수 있는 이론적 근간을 마련하였다.
