Alzheimer's disease (AD) is one of the most common neurodegenerative disease that accounts more than 80% of dementia in the elderly. AD causes the progressive cognitive decline, causing severe suffering for patients and caregivers and puts a great financial burden on the society as well. Key pathogenic process of AD is the accumulation of extracellular amyloid $\beta$ (A$\beta$) and intracellular hyperphosphorylated tau in the brain, while A$\beta$ and tau independent process were also identified. Although extensive research on AD has been made, the pathomechanism of AD is still elusive and there is no treatment that cures or halts the progression of AD.
In this study, I investigated 1) spreading model of tau and A$\beta$ using graph theoretical approach and its potential role in a novel therapeutic strategy for AD, and 2) common genetic variants for cognitive deterioration independent of tau and Aβ using genome-wide association study.
First, I demonstrated that transneuronal spread model well explained the distribution of tau and Aβ while local spread model also explained that of Aβ. In addition, using these spread models, I identified spreading hubs and verified that these hubs were energy efficient control nodes for restoring the AD brain state to the clinically normal state using the network control theory.
Next, I identified 6 novel single nucleotide polymorphism (SNPs) related to cognitive decline, which is not explained by Aβ and tau. Moreover, I discovered that these SNPs were associated with glutathione metabolism, providing new treatment strategy of AD such as anti-oxidative agents to individuals with these SNPs.
To summarize, by leveraging a wide range of methodologies including neuroimaging, genetics, and bioinformatics, I endeavored to identify the pathomechanism that are common (Aβ and tau) as well as distinct from typical AD pathogenesis. I expect that findings from this study would expand the knowledge of AD and provide novel therapeutic strategies.
알츠하이머 치매는 노인성 치매의 80% 이상을 차지하는 가장 흔한 퇴행성 뇌질환으로 점진적인 인지기능 저하를 일으켜 환자뿐만 아니라 가족에게까지 심각한 고통을 안겨준다. 범세계적인 인구고령화에 따라 알츠하이머 치매 환자가 늘고 있어 경제적 그리고 사회적으로 큰 문제가 되고 있다. 알츠하이머 치매의 중요한 병리현상은 세포외 아밀로이드 베타와 세포내 과인산화된 타우의 뇌안의 침착으로 병의 발생과 진행에 관련이 있다. 하지만 아밀로이드 베타와 타우 단백질과 독립적인 병리과정도 확인되어 알츠하이머 치매에는 복잡한 병태생리가 있다고 생각되고 있다. 하지만 아직까지 알츠하이머 치매의 정확한 병태생리가 밝혀져 있지 않고 치료법 또한 개발되지 않았다.
본 연구에서는 먼저 휴지기 기능적 자기공명영상과 그래프 이론을 이용하여 아밀로이드 베타와 타우 단백질이 어떤 모델을 통해 뇌 전체로 확산되어 나가는지 밝히고자 하였으며 확산모델을 통해 거꾸로 뇌내 아밀로이드 베타와 타우 단백질을 줄일 수 있는 새로운 치료 전략을 제시하고자 하였다. 그 다음으로 전장유전체상관성분석을 통해 아밀로이드 베타와 타우 단백질과 독립적으로 인지기능 저하를 일으키는 유전인자를 찾고자 하였고 이를 통해 알츠하이머 치매의 새로운 병리과정과 치료법을 제시하고자 하였다.
먼저, 본 연구자는 아밀로이드 베타와 타우 단백질이 신경간 확산모델을 통해 뇌 전반에 퍼져 있음을 확인하였고 아밀로이드 베타는 타우 단백질과 다르게 국소 확산모델도 함께 따름을 확인하였다. 또한 두 확산모델을 통해 찾은 확산 허브를 컨트롤 노드로 사용했을 때 에너지 효율적으로 뇌내 병인 단백질을 제거 할 수 있음을 네트워크 제어 이론을 통해 확인하였다. 다음으로, 본 연구자는 전장유전체상관성분석을 통해 아밀로이드 베타와 타우 단백질과 독립적으로 인지기능 저하를 일으키는 6개의 새로운 단일염기변형을 찾았다. 새롭게 발견된 단일염기변형은 글루타치온 대사와 연관성이 있었으며 이 결과를 통해 단일염기변형이 있는 환자에게서 항산화치료가 인지기능에 도움을 줄 수 있다는 가능성을 제시하였다.
요약하면, 본 연구자는 뇌영상분석, 그래프이론, 제어이론, 유전자분석, 그리고 생물정보 분석을 통해 알츠하이머 치매의 병태생리를 연구 하였다. 다양한 방향을 통해 얻은 연구 결과들은 알츠하이머 치매의 병태생리의 이해와 새로운 치료 전략의 발견에 중요한 단서를 제공해줄 수 있을 것으로 생각된다.
