A psychotherapeutic regimen that uses alternating bilateral sensory stimulation (ABS) has been used to treat post-traumatic stress disorder in humans. However, the neural basis that underlies the long-lasting effect of this treatment—described as eye movement desensitization and reprocessing—has not been identified. Here we describe a neuronal pathway driven by non-invasive visual stimulation of the superior colliculus (SC) that mediates persistent attenuation of fear. We successfully induced a lasting reduction in fear in mice by pairing visual ABS with conditioned stimuli during fear extinction. Among the types of visual stimulation tested, ABS provided the strongest fear-reducing effect and yielded sustained increases in the activities of the SC and mediodorsal thalamus (MD) throughout the extinction session. Optogenetic manipulation revealed that the SC–MD circuit was necessary and sufficient to prevent the return of fear. ABS suppressed the activity of fear-encoding cells and stabilized inhibitory neurotransmission in the basolateral complex of the amygdala (BLA). The direct MD-BLA projection formed a novel feedforward inhibitory circuit that was required for the observed long-lasting suppression of fear. Together, these results reveal the neural circuit that underlies an effective strategy for sustainably attenuating traumatic memories.
외상 후 스트레스 장애의 치료에는 심리치료 기법이 주로 사용되고 있다. 안구운동 민감소실 및 재처리 요법(Eye movement desensitization and reprocessing)에서는 양측성 빛자극(Alternating bilateral stimulation)을 이용하여 정신적 외상을 치료하는데, 이에 관여하는 신경회로 및 기작은 밝혀지지 않았다. 이 연구에서는 위둔덕(Superior colliculus)의 빛 자극으로 활성화되어 지속적인 공포 감소를 일으키는 신경 회로를 밝혀내었다. 생쥐의 공포기억 소거 과정에서 양측성 빛자극을 제시하면 위둔덕 및 중앙 내측 시상핵(Mediodorsal thalamic nucleus) 신경세포의 발화가 증가하였으며, 이 신경세포들의 활동은 지속적인 공포 반응 감소 효과와 상관관계를 보였다. 광유전학 기법을 이용한 실험에서는 중앙내측시상핵으로 향하는 위둔덕 신경신호 전달이 지속적인 공포 감소의 필요충분 요소임을 보여주었다. 양측성 빛자극은 기저외측 편도체(Basolateral complex of the amygdala)의 공포반응을 조절하는 신경세포의 활동을 억제하였으며, 기저외측 편도체로 향하는 억제성 신경신경신호전달의 빈도가 오랫동안 증가하는 것을 확인하였다. 양측성 빛자극에 의한 공포기억 감소에는 중앙 내측 시상핵에서 기저외측 편도체로 향하는 신경 전달이 필요한 것으로 확인되었는데, 새로 발견된 중앙 내측 시상핵으로 부터의 피드포워드 억제(Feedfoward inhibition)가 양측성 빛자극에 의해 강화되는 것으로 나타났다. 이 연구 결과는 심리 치료 효과를 매개하는, 지속적으로 공포 반응을 억제할 수 있는 새로운 신경회로를 밝혀내었으며, 이를 통해 효과적인 외상 후 스트레스 장애 치료를 위한 새로운 연구 대상을 제시한다.
