Since the level of the delivery of healthcare differs from place to place in each country, researches have been conducted to establish the measurement which can comprehensively assess it. As a result of those endeavours, healthcare spatial accessibility has been used a lot to evaluate the availability of and the geographical accessibility to healthcare resource at each region. However, most researchers have applied previous works to other situations uncritically or added few modifications without verification, because it is hard to obtain patients' data needed to substantiate their suggestions. In this paper, I analyzed drawbacks of the existing measurements and proposed the new one including three improvements. First, relative travel time weights are defined to represent the relative value of healthcare facilities which a patient would perceive at each region. Second, absolute travel time weights are established to describe the mobile convenience of patients in common. Lastly, I corrected the factor in reflecting competition between patients since it has been applied in a wrong way. Besides, all improvements were verified or created from utilization data on the Korean emergency medical system. Consequently, I evaluated spatial accessibility of the Korean emergency medical system by the improved metric and defined deprivation regions.
대부분의 나라에서 지역별 의료 공급의 차이가 발생하는데, 이를 종합적으로 평가할 수 있는 지표에 대한 연구가 많이 진행되어 왔다. 그 결과물로, 의료 공간적 접근성 지표가 많이 사용되는데, 이 지표는 일반적으로 해당 지역의 의료 자원의 이용 가능성과 의료 자원까지의 접근 용이성을 나타낸다. 그러나 관련 자료를 얻는 것이 매우 어렵고, 그에 따라 실제 자료와의 검증이 어렵기 때문에, 많은 기존 연구들은 이전 결과물을 비판없이 적용해보거나, 논리적으로 만들어낸 약간의 개선 사항을 적용해보는데 그 한계점이 존재한다. 이번 연구에서는 기존 의료 공간적 접근성 지표의 문제점을 분석하고, 세 가지 개선사항이 반영된 새로운 의료 공간적 접근성 지표를 제안한다. 세 가지 개선사항 중 첫 번째로, 지역별로 환자들이 느끼는 의료기관의 상대적인 가치를 나타내기 위해 상대적 이동시간 가중치를 정의한다. 두 번째로, 환자들에게 보편적으로 적용되는 이동 편의성 가중치 (절대적 이동시간 가중치)를 정의한다. 세 번째로, 의료 자원의 이용 가능성을 나타내기 위해 잘못 적용된, 환자들 간의 경쟁요소를 바로 잡는다. 덧붙여, 이 세 가지 개선사항을 한국 응급의료서비스의 실제 환자 방문 자료를 이용하여 검증하거나, 도출한다. 최종적으로, 새로운 의료 공간적 접근성 지표를 통해, 한국 응급의료서비스 취약지를 정의한다.
