Clinical trials in recent years have tested the combined anti-tumor effects of radiotherapy and anti-VEGF-A therapy, but its clinical benefit of increased overall survival has been proven to be insufficient. In this study, I rationalized and investigated the benefit of combining VEGF-Grab, an anti-angiogenic drug that inhibits VEGF-A and PlGF, with radiotherapy for anti-cancer therapy. By in vitro and human sample analyses, I showed that PlGF is increased in response to radiotherapy. Then, using a syngeneic mouse tumor model, I demonstrated the most potent anti-tumor effects of combined VEGF-Grab and radiation treatment, which was achieved partially by tumor vessel normalization and polarization of tumor-associated macrophages to anti-tumorigenic M1-type. Finally, I showed that VEGF-Grab is not only efficacious but also more effective than a previous developed anti-angiogenic drug, VEGF-Trap, in human xenograft model. In conclusion, this study suggests VEGF-Grab as a viable therapeutic option in the context of inhibiting secondarily activated pathways, specifically in combination with radiotherapy.
현재 임상에서는 방사선 치료와 함께 VEGF-A 억제 요법을 병행하는 시험이 진행되고있으나 두 가지 치료를 동시에 적용하더라도 환자의 생존율이 증가하지 않는 경우가 많이 보고되고있다. 본 연구에서는 VEGF-A와 PlGF를 동시에 억제하는 것으로 알려진 VEGF-Grab을 방사선치료와 병행하는 치료법에 대한 과학적 근거를 제시하고 그 효과를 임증하였다. 세포 실험과 환자 혈액 검체를 이용한 실험을 통해 방사선 치료의 결과로 PlGF의 발현양이 증가함을 확인한 후 마우스 동계(同系) 종양 모델을 이용해 VEGF-Grab과 방사선 치료를 병행할 경우 암 치료 효과가 증진되는 것을 확인하였고, 이는 VEGF-Grab에 의한 암 혈관 생성 억제 효과 뿐만 아니라 암 혈관 정상화와 종양관련대식세포의 유입 및 성질 변화를 통해 이루어짐을 입증하였다. 나아가 인간 유래 암 세포를 이용한 마우스 이종(異種) 종양 모델 실험으로 VEGF-Grab이 인체 내의 종양에서도 효과적일 수 있음을 시사하였다. 종합적으로, 암환자에게 방사선 치료와 함께 VEGF-A와 PlGF를 모두 억제할 수 있는 VEGF-Grab을 사용하는 것이 암치료에 더욱 효과적일 것으로 기대된다.
