The problems of the current health care system in Korea and its health policy fall in three principal categories : problems of effectiveness, efficiency and equity. This study has grown out of an increasing concern that the health policy in Korea has misplaced emphasis among different means of improving health. A model of health production is formulated in this paper. The model postulates that four major factors determine the production of health. They are socio-economic, health services, environmental and genetic factor. Each factor is divided further into several variables as shown below.
Socio-economic factor ; income, education
Health services ; health input (health personnel, facility)
health input-mix (labor-mix, capital-labor mix)
Environmental factor ; demographic variables
modernization variables
Genetic factor is excluded from this analysis because there are no data about it.
The age-sex adjusted death rate is selected as negative index of health. The results from regressions employing the cross-sectional data of cities and guns in Korea as follows.
(1) Education is the most important factor in determining death rate. Thus an investment in education would contribute to the improvement of health more than anything else.
(2) A higher income is related to a better health and a higher income growth rate is related to a lower level of health.
(3) In order to improve nation's health, we have to establish many medical facilities regardless of their types and increase and improve the primary health care system.
(4) Health facility is a more influential variable in determining mortality than health personnel.
(5) Among health facilities, hospitals and clinics are most important one in influencing death rate.
(6) Among health personnel, medical doctors exert more influence on mortality than any other professions.
(7) Korea will be the aged society in 15-20 years. Therefore, we must be concerned about medical welfare rather than new medical technology.
韓國의 保健醫療體系 및 保健政策이 가진 問題點은 效果性, 效率性, 均衡性의 세가지로 나눌 수 있다. 本 硏究는 韓國의 保健政策이 여러 國民健康改善策들 간의 優先權에서 잘못 强調를 두고있다는 点에서 着眼되어진 것이다.
本硏究에서는 健康에 影響을 미치는 諸要素를 規定하는 健康生産 모델을 開發하고 그 要素들의 影響力을 살펴보아 優先權의 確立에 基礎가 되게하여 保健資源의 最適配分에 도움이 되고자 함과 동시에 先進國을 對象으로 한 旣存의 硏究結果가 우리나라의 現實狀況에서는 어떻게 修正되어야 하는 하는가를 밝히고자 한다.
保健醫療의 消費者들은 醫療서어비스 供給者가 提供하는 醫療서어비스 를 단순히 消費하는 機能만을 가지는 것이 아니라 그 醫療서어비스를 받아들여 健康을 生産하는 重要한 機能도 가진다.
健康生産이란 窮極的으로 個人 自身이 構成하며 決定하는 것이다.
따라서 各 個人은 企業家와 消費者의 機能을 한꺼번에 遂行하여 自身의 健康生産을 管理함과 동시에 그로 因한 利益을 享有하고 있다.
健康生産者는 市場에 내다팔기 위해 生産을 하여 그 利益을 極大化하려 는 企業家와는 다르다. 그의 行爲는 個人的인 效用極大化法則에 따라 決定되는 것이다. 結局 健康生産者는 健康에 害가 되는 行爲를 自發的으로 할 수도 있는 것이다.
本 硏究의 健康生産 모델에는 健康에 影響을 주는 要素들, 즉 社會. 經濟的 要素, 保健 서어비스, 環境的 要素, 遺傳的 要素의 네가지가 規定되어 있으며 위의 要素들은 각각 몇개의 變數들로 細分되어 진다.
本 硏究에서는 健康의 負的指數로서 年齡別·性別 補完死亡率을 採擇하였으며 資料는 1978년 韓國의 市郡別 資料를 이용하였다. 또한 Simultaneous equation 問題를 解決하기 위하여 two-stage least squares method를 導入하였으며 이 方法을 通해 各要素들의 純粹效果分析(Pure effect analysis)도 하였다. 그 結果中 重要한것은 다음과 같다.
ⓛ 敎育이 死亡率을 決定하는데 가장 重要한 要素이다. 따라서 敎育에의 投資는 다른 무엇보다도 健康改善에 크게 寄與할 것이다.
② 所得의 增加는 健康向上에 關係가 있으며 높은 所得增加率은 健康水準의 下落을 가져온다.
③ 國民健康을 向上시키기 위해서는 어떠한 形態이든지 醫療機關을 많이 設立해야하며 一次診療券도 擴大시켜야 한다.
④ 醫療施設이 醫療人力보다 死亡力決定에 더 影響力있는 變數이다. 醫療施設中에서는 病院과 醫院(hospitals and clinics)이, 醫療人力中에서는 醫師가 影響力이 크다.
⑤ 人口의 年齡構造 趨勢를 볼때 韓國도 15年내지 20年後에는 先進國과 마찬가지로 高齡化 社會가 될 것이다. 따라서 새로운 醫療技術의 開發 및 導入보다는 醫療福祉에 關心을 기울여야 하겠다.
