Rheumatoid arthritis (RA) is a chronic autoimmune disease that causes progressive joint inflammation and destruction of synovial tissues. Because RA is a multi-factorial disease, both genetic and environmental factors contribute to RA susceptibility and multi-genomic loci have been shown to be linked to RA susceptibility. HLA-DRB1 locus and several genes in non-HLA regions were reported to be associated with RA. However, these genes cannot totally explain genetic susceptibility of RA. In the present work, three types of case-control studies were performed to identify additional genetic polymorphisms associated with RA using genomic DNA from 1316 Korean RA patients and 1006 Korean controls.
In the first candidate gene approach, 209 single nucleotide polymorphisms (SNPs) from 87 candidate genes in non-HLA region were investigated for identification of putative RA-associated SNPs by estimating allele frequency difference between genomic DNA pools of 1032 cases and those of 688 controls. Allele frequency differences of 7 SNPs in different 7 genes were higher than 0.045. Among these 7 SNPs, one SNP named as RA243 (rs1059703) located in X-chromosome showed significant allelic associations with RA when total RA and control subjects were individually genotyped and analyzed for RA associations. Two major haplotypes consisted of 6 SNPs containing the RA243 in a linkage disequilibrium (LD) block which ranges from 5’ downstream region IRAK1 to middle of MECP2 in X-chromosome were also associated with RA susceptibility in total subjects and subgroups stratified by genders. These two IRAK1/MECP2 haplotypes were also shown to be associated with rheumatoid factor positivity. No significant difference of mRNA expression levels of two IRAK1 isoforms and MECP2 was detected between two IRAK1/MECP2 haplotypes implying no existence of expression-regulatory SNPs. Because nonsynonymous SNPs do not exist on exonic regions of MECP2 in dbSNP list, two RA-associated nonsynonymous SNPs, rs1059702 and rs1059703 are strong candidate for real RA-causal variants.
In the second association study, SNPs identified in previous genome wide association studies in Caucasians were tested for replicated RA associations in Koreans. 12 SNPs identified with genome-wide scans and 12 additional tag SNPs in IL2RB, OLIG3-TNFAIP3, PTPN22, and TRAF1/C5 which were reported to showed consistent RA associations in different Caucasian cohorts were analyzed. None of the SNPs tested was associated with RA susceptibility, except for rs7021206 in TRAF1 intron 3 (P = 0.0032) and documented rs6457617 in HLA ($P = 4.6 \times 10^{-35}$). Association of the rs7021206 was positive in patients seropositive for rheumatoid factor (P = 0.0051) or anti-cyclic citrullinated peptide autoantibody (P = 0.0062). However, Koreans were negative for the association of rs3761847 in the TRAF1-C5 intergenic region previously reported in Caucasians. Linkage disequilibrium between rs3761847 and rs7021206 was not as high in Koreans ($r^2$ = 0.37) as in Caucasians ($r^2$ = 0.67), explaining for the non-association of rs3761847 in Koreans. Accordingly, RA susceptibility was localized to an extended haplotype marked by rs7021206 rather than by rs3761847, and SNPs highly correlated with rs7021206 ($r^2$ ≥ 0.81) extend from rs1953126 in the PHF19-TRAF1 intergenic region to rs2900180 in the TRAF1-C5 intergenic region, spanning 66 kb. Within and around TRAF1 excluding PHF19 and C5, SNPs highly correlated with rs7021206 are associated with RA in both Asians and Caucasians, and possibly correlated with causative variations, but not those correlated with rs3761847.
The final candidate gene approach focused on identifications of the secondary RA-susceptible loci in HLA-region. For adjustment of effects of HLA-DRB1 alleles, RA-association of each HLA-DRB1 was re-evaluated using relative predisposition effects methods. As a result, 8 alleles $(DRB1 ^*0101, ^*0401, ^*0404, ^*0405, ^*0410, ^*0901, ^*1001, and ^*1502)$ were classified as RA-risk alleles and 6 alleles $(DRB1 ^*1301, ^*1302, ^*1403, ^*1405, ^*0701, and ^*0802)$ as RA-protective alleles. Based on previous study results that HLA-class III region includes RA-susceptible genetic polymorphisms which is independent of HLA-DRB1, the rs2071592 in NFKBIL1 and additional 17 SNP s in HLA-class III region were tested for associations about RA. Among 18 SNPs, 16 SNPs showed significant genotypic associations with RA-susceptibility (P < 1.8×$10^{-3}$). However, only one SNP, rs2763979 in 933bp upstream of HSPA1B was shown to be associated with RA after adjustment for HLA-DRB1 alleles by multivariable logistic regression analysis (P = 0.04), implying that RA associations of the rs2763979 is independent of HLA-DRB1, and the other SNPs are in extended LD effects with HLA-DRB1 alleles. Because the rs2763979 is highly correlated with three other SNPs in C6orf48 and LY6G6C ($r^2$ > 0.82) associations between these SNPs and RA need to be tested so as to identify a real RA-associated gene in HLA-class III region.
대표적인 만성 자가면역 질환인 류마티스 관절염은 유전적 요소와 환경적 요소가 복합적으로 작용하는 다인성 질환으로서, 원인 유전자는 인간 유전체 내에 여러 곳에 존재한다. 유전 역학 연구를 통해 HLA-DRB1 유전자 및 몇 개의 비-HLA 유전자가 류마티스 관절염과 연관성이 있는 것으로 밝혀 졌지만 해당 질환의 유전 원인을 전체적으로 설명하기 위해서는 추가적인 연관성 유전자의 발굴이 필요하다. 본 연구에서는 1316명의 한국인 류마티스 관절염 환자 및 1006명의 정상인 환자의 유전체를 이용한 세가지 타입의 사례-대조군 연구를 통하여 신규 류마티스 관절염 유전자 발굴하였다.
첫째로 기존에 연관성이 밝혀지지 않은 비-HLA 부위의 87개 후보 유전자의 209개 단일 염기 다형성 (SNP) 에 대해서 연관성 연구를 수행하였다. 1032명의 류마티스 환자 유전체 pool과 688개의 정상인 유전체 pool의 대립 유전형질의 빈도수의 차이가 4.5% 이상 나는 7개의 SNP를 발굴하였고 전체 샘플에 대한 연관성 테스트 결과 7개의 SNP중에서 X 염색체에 위치한 한 개의 SNP (rs1059703)이 유의미한 연관성을 보였다 (P = 0.0046). rs1059703이 위치한 IRAK1/MECP2 부위의 추가 SNP을 분석한 결과 rs1059703을 포함한 6개의 SNP이 서로 강한 상관관계를 나타냈으며 6개의 SNP이 구성하는 두 개의 주요 일체배형 (haplotype)이 류마티스 관절염과 강한 연관성을 보임을 확인하였다. 해당 일체배형은 류마티스 관절염의 주요 분자 마커인 류마티스성 인자 (Rheumatoid factor) 유무와도 연관성을 나타내었다. IRAK1/MECP2의 일체배형과 IRAK1 또는 MECP2의 발현양과의 상관관계를 각각의 일체배형의 동형을 가진 B 세포의 mRNA 정량을 통해서 분석한 결과 상관관계가 없음을 보였다.
둘째로 기존에 유럽인종에서의 전-유전체 연관성 연구로 밝혀진 단일 염기 다형성에 대하여 한국인에서의 재현성 여부를 테스트하였다. 네 개의 연구 그룹에서 발표한 12개의 류마티스 관절염 연관성 단일 염기 다형성에 대해 연관성을 분석하였고, 추가적으로, 유럽인종에서 재현성 있는 연관성 결과를 보인 네 개의 loci인 IL2RB, $OLIG3^~$ TNFAIP3, PTPN22, and TRAF1/C5 에 위치한 12개의 tagSNP에 대해 연관성을 분석하였다. 총 24개의 SNP 중에서 HLA-DRB1 근처에 위치한 rs6457617을 제외하고 TRAF1 유전자의 인트론에 존재하는 rs7021206만이 유의미한 연관성을 나타내었으며 (P = 0.0032) 연관성은 류마티스성 인자와 시트룰린화 펩타이드의 존재 여부와 상관이 없음을 보였다. 그러나 기존에 유럽인종에서 연관성을 보인 TRAF1/C5에 존재하는 또다른 SNP인 rs3761847은 한국인에서 연관성을 보이지 않았다. 그 이유는 rs7021206과 rs3761847의 상관관계 계수 ($r^2$)가 한국인에서 0.37인 반면 코카시언에서는 0.67로 높기 때문으로 추정된다. 따라서 실제 류마티스 관절염 감수성 SNP는 rs3761847이 아닌 rs7021206과 강한 상관관계를 가지는 ($r^2 ≥ 0.81) SNP으로 구성된 일체배형에 포함되며 이는 PHF19와 TRAF1 유전자 사이에 존재하는 rs1953126에서 TRAF와 C5 유전자 사이의 rs2900180에 이르는 66kb이르는 유전체 부위에 위치하고 있다.
마지막으로 HLA-부위에 존재하는 HLA-DRB1을 제외한 또 다른 류마티스 관절염 감수성 유전자를 발굴하기 위해 HLA-class III 부위의 대한 류마티스 관절염 연관성 연구를 수행하였다. HLA 부위의 유전자들은 위치에 상관없이 HLA-DRB1 유전자와의 일체배형 효과에 의해 류마티스 관절염과 연관성을 나타낼 수 있다. 따라서 HLA-DRB1의 효과에 대한 보정이 필요하며 이를 위해 HLA-DRB1의 대립 유전 형질을 상대적 소인 효과법 (RPE method)을 이용하여 분류한 결과 류마티스 관절염 위험성 효과 대립형질 8개 $(DRB1 ^*0101, ^*0401, ^*0404, ^*0405, ^*0410, ^*0901, ^*1001, and ^*1502)$와 보호효과 대립형질 6개$(DRB1 ^*1301, ^*1302, ^*1403, ^*1405, ^*0701, and ^*0802)$가 발굴되었다. 해당 HLA-DRB1 대립 유전 형질에 대한 유전형을 모두 보정하는 방식으로 기존에 일본인에서 연관성이 알려진바 있는 NFKBIL1 유전자의 rs2071592를 포함하는 HLA-class III부위의 18개의 SNP에 대한 연관성 분석을 실시한 결과 17개의 SNP은 HLA-DRB1에 대한 보정전에 류마티스 관절염과의 강한 연관성을 보였지만 (P < 1.8 ×$10^{-3}$), HLA-DRB1 에 대한 보정 후에는 HSPA1B 유전자의 933 base pair 앞쪽에 위치한 rs2763979이 유일하게 유의미한 연관성을 보였다 (P = 0.04). rs2763979는 C6orf48 와 LY6G6C 에 위치한 세 개의 다른 SNP과 강한 상관관계를 가지기 때문에 ($r^2$ > 0.82) 실제 류마티스 관절염 원인 유전자를 밝히기 위해서는 이들 SNP에 대한 분석이 추가적으로 필요하다.
