Breast cancer is one of the major causes of mortality increase to middle aged women, especially in developed countries. Mammography associated with clinical breast examination is the most effective method for early detection of breast cancer. Clustered microcalcifications on mammograms are an important sign in the detection of breast cancer. Mammogram interpretation has been performed by radiologists by visual examination of the films for the presence of abnormalities that can be interpreted as cancerous changes. The computer-aided diagnosis (CAD) will be useful to increase the diagnosis sensitivity of radiologists. The CAD can be categorized into three groups, such as the image enhancement, the detection of suspicious lesions, and the classification of suspicious lesions as benign or malignant. This thesis proposes three effective CAD methods; one is for the adaptive enhancement of mammographic images, another is for the detection of clustered microcalcifications, and the other is for the classification of clustered microcalcifications.
The proposed adaptive enhancement is based on the first derivative and the local statistics. The adaptive enhancement method consists of three processing steps. The first step is to remove the film artifacts which may be misread as microcalcifications. The second step is to compute the gradient images by using the first derivative operators. The third step is to enhance the important features of the mammographic image by adding the adaptively weighted gradient images. Local statistics of the image are utilized for adaptive realization of the enhancement, so that image details can be enhanced and image noise can be suppressed. The objective performances of the proposed method were compared with those by the conventional image enhancement methods for a simulated image and the seven mammographic images containing real microcalcifications. The performance of the proposed method is also evaluated by means of the receiver operating-characteristics (ROC) analysis for 78 real mammographic images with and without microcalcifications.
A statistical texture analysis method, called the surrounding region dependence method (SRDM),is proposed for the detection of the clustered microcalcifications on digitized mammograms. The SRDM is based on the second -order histogram in two surrounding regions. This method defines four textural features to classify region of interests (ROIs) into positive ROIs containing clustered microcalcifications and negative ROIs of normal tissues. Comparative studies of texture analysis methods are performed for the SRDM and the conventional texture analysis methods such as the spatial gray-level dependence method (SGLDM), the gray- level run-length method (GLRLM), and the gray-level difference method (GLDM). A three-layer backpropagation neural network is employed as a classifier. The results of the neural network for texture analysis methods are evaluated by the ROC analysis. A segmentation of individual microcalcifications is also proposed to show their morphologies. The performance of the proposed method is also evaluated by using the free-response receiver operating- characteristics(FROC) analysis. In terms of the FROC, the proposed method gives a sensitivity of more than 90% with a low false-positive (FP) detection rate of 0.67 per cropped image.
A shape-based method, which is fully based on the morphological criteria of the clustered microcalcifications, is proposed for the classification of the clustered microcalcifications as benign or malignant. To find the most useful method for the classification of the clustered microcalcifications, comparative studies are performed for the proposed method and the conventional methods such as the wavelet-based method and the texture-based method. These methods are applied to classify ROIs into positive ROIs containing malignant microcalcifications and negative ROIs containing benign microcalcifications. A three-layer backpropagation neural network is employed as a classifier. The results of the neural network for methods are evaluated by the ROC analysis.
유방암은 중년 여성의 사망 율 증가의 주 원인 중에 하나이다. X 선 마모그래피 (X-ray mammography) 는 유방암의 조기 진단을 위한 가장 효과적인 의료 장비로 알려져 있다. 진단 방사선 의사들은 맘모그램 필름상에서 육안 또는 확대경으로 암과 관련된 병소 (lesions)를 찾는다. 따라서 컴퓨터 보조 진단은 진단 방사선 의사의 진단 정확도를 향상시키기 위한 하나의 대안으로 연구되고 있다. 컴퓨터 보조 진단은 3가지 그룹 즉, 맘모그램 영상 강조 (image enhancement), 병소의 검출 (detection), 그리고 병소의 양성 (benign) 또는 악성(malignant)으로의 분류 (classification)들로 나눌 수 있다. 본 논문에서 유방암의 조기 진단을 위한 적응 영상 강조, 군집성 미세석회화 (clustered microcalcifications)의 검출, 그리고 군집성 미세석회화의 분류를 위한 효과적인 컴퓨터 보조 진단 방법을 각각 제안했다. 군집성 미세석회화는 유방암과 관련된 주요 병소 중 하나이다.
맘모그램의 적응 영상 강조 기법은 맘모그램 영상의 1차 미분 (first derivative) 및 국소 통계학 (local statistics)에 기초를 두고 있다. 적응 영상 강조는 3 가지 처리 단계로 구성되어 있는데, 첫 번째 단계는 미세석회화로 오인할 수 있는 필름 흠 (artifact)을 제거하고, 두 번째 단계에서는 1차 미분 연산자를 이용하여 gradient 영상들을 계산한다. 세 번째 단계에서 적응 적으로 가중된 각 gradient 영상을 원 영상에 더함으로써 맘모그램의 중요한 특징, 특히 낮은 대조도 (contrast)를 나타내는 병소를 강조한다. 영상의 국소 통계학은 적응 강조를 실현하기 위해 이용되고 있으며 따라서 영상의 세부 정보 (details)들을 강조하면서 잡음을 억제할 수 있다. 영상 강조 기법의 객관적인 평가를 위한 한 기준, 즉 CIR (contrast improvement ratio)를 제안했다. 종래의 방법들과 객관적인 비교를 위해 실제 병소와 유사한 형태를 정상적인 맘모그램에 삽입한 가상 영상 (simulated image) 및 실제 군집성 미세석회화를 포함한 7개의 맘모그램에 대해 CIR 측면에서 비교 검토하였다. 또한 제안된 영상 강조 기법은 ROC (receiver operating-characteristics) 분석에 의한 임상 평가를 수행하였다.
맘모그램에서 군집성 미세석회화를 검출하기 위해 통계학적인 texture 분석, 즉 surrounding region dependence method (SRDM)을 제안했다. 제안된 SRDM은 두개의 인접된 영역에서의 2차 히스토그램 (second-order histogram)에 기초를 두고 있고 맘모그램의 정상 조직들로 부터 군집성 미세석회화를 포함하고 있는 영역의 검출을 위한 4개의 특징 값이 정의된다. 종래의 texture 분석 방법과 성능 비교를 하였고 three-layer backpropagation 신경 망이 분류기 (classifier)로 이용되었다. 각 texture 분석 방법에 대한 신경 망의 출력 값들은 ROC 분석에 의해 평가되어 진다. 검출된 영역에서 개개의 미세석회화를 검출하기 위한 새로운 분할 방법 (segmentation method)이 제안된다. 이것은 양성 또는 악성 군집성 미세석회화의 식별을 위한 형태학적인 판정 기준으로 사용된다. 또한, 제안된 군집성 미세석회화의 검출 방법은 free-response receiver operating- characteristics (FROC) 분석에 의해 평가되었다. FROC 분석 결과로서, 영상 당 0.67의 위양율 (false-positive rate)에 90% 이상의 민감도 (sensitivity)를 나타내었다.
군집성 미세석회화의 양성 또는 악성으로의 분류를 위한 shape-based method를 제안했고 이것은 군집성 미세석회화의 형태학적인 판정 기준 (morphological criteria)에 기초를 두고 있다. 종래의 분류 방법과 성능 비교를 하였고 three-layer backpropagation 신경 망이 분류기 (classifier)로 이용되었다. 각 분류 방법에 대한 신경 망의 출력 값들은 ROC 분석에 의해 평가되어 진다.
