Various dermatological samples containing liposomes as a drug carrier were prepared, and their effects on drug stability and characteristics for effective topical drug delivery were investigated. Hydrocortisone-21-acetate, an antiinflammatory agent, was used as the model drug.
Being hydrophobic and water-degradable, hydrocortisone acetate was first expected to be unstable in liposomes. However, upon encapsulation in liposomes by the precipitation method, the drug particles were found to be entrapped in the liposomes, as a solid core surrounded by a multilamellar lipid membrane in each vesicle. This structure seemed to have protective effects on the drug by minimizing the amount of water available around the drug particles and reducing the chances of water-induced degradation reactions.
Three types of ointment bases, namely hydrogel, cream, and poly(ethylene glycol) ointment, were prepared, and variations in the ointment formulations were made for the study of the effects of liposomal encapsulation, base materials, and the purity of lipid products on the stability of hydrocortisone acetate. It was found that liposomally encapsulated drug was more stable than free-form drug in an ointment sample. Also, the hydrogel base was found to be the most effective for the drug stability among the three types of bases in spite of its high water content. Moreover, the high viscosity of the hydrogel base seemed to have contributed to the maintaining of the drug stability. It was also found that the lipid product of higher purity provided better effects on the drug stability.
The liposomal hydrogel ointment was found to be advantageous in topical drug delivery from an in vitro test of drug permeation through the pig ear skin and a test of drug release into an aqueous medium. The amount of hydrocortisone acetate that had permeated through the skin membrane in the test was found to be three times less in the case of liposomal hydrogel than in the case of free-drug hydrogel. However, the drug release behavior was almost similar in both hydrogel ointments. The results from the two tests suggest that liposomes played a role in retaining drug molecules within the skin and that they were effective in delivering drug to the skin.
리포솜을 약물전달체로 하는 각종 피부용 연고를 제조하고, 약물의 안정성과 효과적인 국부적 약물 전달을 위한 연고의 특성 등을 관찰하였다. 시험 약물로는 소염제인 초산 하이드로코르티손을 이용하였다. 초산 하이드로코르티손은 소수성을 띠고 수분과 반응하여 변질되는 성질이 있기 때문에 리포솜 내에서 불안정할 것으로 생각되었다. 그러나, 침전법으로 리포솜을 제조하였을 때, 제조된 리포솜은 약물 입자를 중심으로 다중층의 지질막이 입자를 감싸고 있는 형태를 가진 것으로 나타났다. 이런 형태는 약물이 수분에 노출되는 것을 최대한 막아 수분과 반응이 일어날 가능성을 줄이는 것으로 보인다.
하이드로젤, 크림, 폴리에틸렌 글라이콜 연고 등의 세 가지 유형의 연고를 제조하고 제제의 성분을 변수로 하여, 리포솜과 연고 기제와 지질의 정제도 등이 초산 하이드로코르티손의 안정성에 미치는 영향을 관찰하였다. 그 결과, 리포솜에 봉입된 약물이 봉입되지 않은 약물에 비해 연고 내 안정성이 더 좋은 것으로 나타났다. 또한, 하이드로젤 기제가 높은 수분 함유량에도 불구하고 세 가지 연고 기제 중 가장 약물 안정에 효과가 있는 것으로 나타났다. 이런 특성을 나타내는 원인에는 하이드로젤 기제의 높은 점도도가 부분적으로 기여하는 것으로 생각된다. 또한, 잘 정제된 지질 제품으로 제조된 리포솜이 약물 안정성을 높이는 것으로 관찰되었다.
돼지 피부를 이용한 약물 투과 실험과 약물 방출 실험에서, 리포솜을 함유한 하이드로젤 연고는 리포솜이 없는 연고보다 약물 전달 특성이 더 좋은 것으로 나타났다. 피부 투과 실험에서는 약물이 리포솜에 봉입되어 있을 때 피부를 완전 통과하여 빠져나오는 양이 봉입되지 않았을 때보다 세 배나 적은 것으로 나타났고, 방출 실험에서는 별 차이 없는 것으로 밝혀졌다. 이 결과는 리포솜이 약물을 피부 내에 머물게 하여 피부 세포에 효과적으로 약물을 전달하는 역할을 한다는 것을 의미한다.
